Item 8 Commodes Analysis/Action – Whilst the position has improved over the past twelve months there continues to be sporadic failures from both the meridian audit and the internal audit process (latter improved recently). Meridian now sends a real time alert direct to a specified group when a commode inspection fails; if any ward fails the commodes audit, they are automatically monitored on a daily basis until the Matron is assured that the issues are addressed (i.e. as evidenced by consistent non-failure of commode inspections). 21

These have been categorised by type to provide assurance that themes and trends have been identified and that improvement programmes are in place. During the same period the Trust received 36 serious and significant complaints, shown as Annex 2. Quality Impact Assessment This report provides assurance that progress is being made in meeting the Trust’s objective of providing safe, effective and personal care to patients. Financial Impact Assessment Potential loss of income if Trust fails to meet CQC requirements and safety obligations. Regulatory Impact Assessment Applicable to all CQC regulations relating to Safety & Quality. Key Recommendations The Board are asked to note the contents of the report.
