Item 8 Falls The essence of care audit assesses the following: • Compliance with Morse assessment • Compliance with up-to-date EPUAP assessment Analysis/Action - This suggests that assessments of risks of falls are being undertaken on a reliable basis. However, if does not tell us how these risk assessments have informed the care plan or if the correct preventative measures have been put in place to reduce the risk of falls in hospital. The Safe Care Group will consider a paper in July proposing a revised approach to measurement (outcome focussed) and a re-launch of the Falls reduction package across our wards. Nutrition & Hydration The essence of care audit assesses the following: • Compliance with MUST assessment • Where needed has a food chart been completed? Analysis/Action - The newly formed Nutrition task and finish will review all feedback regarding nutrition and develop an improvement plan to address patient feedback results, reduce patient complaints and improve use of MUST assessment. The question in the local inpatient survey will also be revised to ensure that vulnerable patients are being asked the question; the current results indicate that the wrong patients are being asked if they received help and thus the giving false assurance. 19
Item 8 Observations The essence of care audit assesses the following: • Have observations been performed at the required frequency • Has there been a red trigger in the last 24hrs? • What is recorded about escalation actions trigger? Analysis/Action – these results would suggest that Observations are being done when required and that triggers are being escalated. However, a recent spot audit undertaken on one day suggested concerns about a lack of escalation. That said, different measures were used during the spot audit and as a consequence, a review is underway. and a further spot audit undertaken in July. 7.2 Cleanliness Audits 1951 audits have been completed up to 18 June 2013. The environment audit addresses a number of cleanliness issues; the key issue being the cleanliness of commodes as part of infection prevention and control. These audits include regular ‘Peer review’. 20
