Every reported incident is classified based on whether there was any harm to the patient and, if so, the extent of that harm. The previous graph (4.1) illustrates an increasing reporting rate which, when combined with the above graph, again suggests that the proportion of incidents resulting in harm to patients is stable in spite of increasing reporting. Each month, a snapshot audit is undertaken on every inpatient on a single day. The audit records the prevalence of 4 different types of harm events: • Pressure Ulcers • Catheter Associated Urinary Tract Infections • DVT and PE • Falls The audit also records whether the harm event happened prior to or during admission. PHT is consistently submitting audits for the majority of inpatients and is recognised regionally for this.
