JOB DESCRIPTION<br />

The Service: 20 years ago, Exeter psychiatric rehabilitation services were well organised and well thought of<br />

but subsequently fell into decline and became either isolated and unsupported or were dispersed. With the<br />

inception of the Devon Partnership Trust and pursuit of the implementation of the NSF for Adult Mental Health<br />

(1999), coupled with thinking of clinical services as interactive systems, there was a renewed emphasis on<br />

developing a comprehensive and responsive rehabilitation service. This was both to meet the needs of its core<br />

patient group and through that the needs of a service as a whole. The appointment of a full time rehabilitation<br />

consultant in 2003 was a significant step in consolidating and taking forwards service developments that began<br />

2 years previously. The service continued to grow and develop by adding a linked community team and then<br />

by growing connections with residential care providers. The service has been a leading area in Devon for the<br />

development of recovery orientated practice.<br />

The service currently comprises of two separate but connected areas of work – an inpatient, residential<br />

rehabilitation service for Devon and a community rehabilitation team for Exeter. Medical and psychology staff<br />

are shared across both teams and there are aspects of joint work and planning. The teams have separate<br />

nursing and OT staff and have different managers. The service is going through a process of re-provisioning<br />

currently, moving towards an integrated Residential and Community team.<br />

The StR Role: The StR is an apprenticeship role, learning rehabilitation psychiatry and at the present stage of<br />

service development, participating also in how to redesign a service shape and remodel it, taking advantage of<br />

new service developments and the complex process of re-commissioning existing ones. The StR would shadow<br />

the consultant clinically and in service development and planning processes. He or she would also take up<br />

specific projects and areas or responsibility as available. There will be ample opportunities to develop the<br />

whole range of specialist competencies in line with the specialist curriculum and the post holder will work<br />

across inpatient, community and out of area environments.<br />

Facilities: Russell Clinic is an inpatient ward (16 beds) at Wonford House Hospital, linked to a dedicated<br />

community rehabilitation team for Exeter. The ward is the main provider of residential rehabilitation for<br />

Devon. We also work with providers of residential care and have close relationships with providers in Exeter.<br />

Russell Clinic has the unique distinction within the trust of being awarded the AIMS (Accreditation of Inpatient<br />

Mental Health Services) by the Royal College of Psychiatrists in July 2011; AIMS is a standards based<br />

accreditation service designed to improve quality of care in psychiatric wards.<br />

Structures: There are a number of organisational structures though which clinical leadership, service<br />

governance and guidance on service developments and design are conducted. This included the Psychosis &<br />

Recovery Forum, the Rehabilitation Core Group, Multidisciplinary team supervision, referral and concerns<br />

meeting and 3 times a week inpatient hand-overs on Russell Clinic. In addition both residential and community<br />

teams have regular team and service development away days and we have close networking relationships with<br />

MIND and Rethink and particularly with Recovery Devon. We also have close working links with service user<br />

and carer groups.<br />

Population: Russell Clinic serves the same area as Devon Partnership Trust, i.e. 850,000.<br />

Community Rehabilitation Team serves only Exeter, with a population of 150,000 approximately.<br />
