The Future—Dr Martin Davis, Associate Dean for Faculty Development<br />

The <strong>Deanery</strong> programme<br />

for Supervisor<br />

Training has been<br />

adapted in response to<br />

feedback from Supervisors<br />

attending the courses.<br />

The changes have<br />

reduced the amount of<br />

time to complete training<br />

and adjusted content<br />

to make it more applicable<br />

to your role and attempts<br />

to avoid duplication.<br />

It remains necessary<br />

for all named supervisors<br />

to have completed<br />

the required<br />

training by the end of<br />

2013. Completion can<br />

be achieved by attending<br />

the courses the<br />

<strong>Deanery</strong> are running or<br />

providing evidence that<br />

you have completed<br />

equivalent training elsewhere.<br />

The details for<br />

the courses and providing<br />

evidence of equivalent<br />

are on the SWPD<br />

website.<br />

A number of Supervisors<br />

have enquired<br />

about the necessity for<br />

updating training and<br />

what format this would<br />

take. Once the initial<br />

training has been undertaken<br />

there is no<br />

expectation that individuals<br />

will need to<br />

repeat these courses.<br />

There will be a necessity<br />

to provide evidence<br />

of on-going engagement<br />

with the training process<br />

to ensure that your<br />

knowledge and expertise<br />

in postgraduate<br />

medical education is<br />

updated. The evidence<br />

for this can be tailored<br />

to meet the individual<br />

Supervisors needs e.g.<br />

attendance at national<br />

specialty meeting related<br />

to this topic, attendance<br />

at <strong>Deanery</strong> organised<br />

sessions on specific<br />

topic etc. The <strong>Deanery</strong><br />

will not be prescriptive<br />

about this, but will require<br />

evidence of engagement<br />

on a regular<br />

basis, consistent with<br />

the job role you are undertaking.<br />

More detail<br />

on this will follow later<br />

in the year.<br />

The recognition and<br />

consequent approval of<br />

the trainer role in medical<br />

education is necessary<br />

to facilitate the provision<br />

of high quality<br />

postgraduate medical<br />

education. It is a priority<br />

of the GMC and the<br />

<strong>Deanery</strong>. If successfully<br />

implemented it should<br />

lead to an acknowledgement<br />

by your employer<br />

of its importance. More<br />

detail of this can be<br />

found on the GMC website.<br />

School Highlight—Paediatrics<br />

The School of Paediatrics has seen<br />

a consolidation of working processes<br />

with continuity provided by<br />

Michael Quinn as Head of School<br />

and Kumar Yadlapalli as Training<br />

Programme Director over the last<br />

4 years. These have included introduction<br />

of annual within School<br />

appraisals of the Trust RCPCH<br />

tutors and TPD. One of the outcomes<br />

of these has been agreement<br />

by the <strong>Deanery</strong> to appoint an<br />

additional TPD to support the ST1<br />

-3 training programme. The<br />

School continues to recruit enthusiastic<br />

and motivated trainees into<br />

the 8 year run through Paediatric<br />

programme. We conduct a local<br />

appointment process in Plymouth<br />

with a ratio of 2 applicants for<br />

every ST1 post equivalent to the<br />

national average. Following feedback<br />

from the Associate Deans for<br />

assessment about trainees’ perception<br />

of the ARCP process, we<br />

developed a change of approach in<br />

which each panel member was<br />

given responsibility for assessing<br />

the e portfolios of trainees prior to<br />

the panel review. Improved representation<br />

of all Trusts on the<br />

ARCP panel allowed for more<br />

time to give formative feedback<br />

having addressed the “tick-box”<br />

components of the exercise beforehand.<br />

This was noted by the external<br />

advisor from Wales who provided<br />

a very positive report on the<br />

process. The trainees appreciated<br />

the opportunity to discuss career<br />

progression. Our College has<br />

introduced a GMC approved pe-<br />

nultimate year assessment<br />

(START or Specialty Trainee<br />

Assessment of Readiness for Tenure)<br />

in ST7 to determine readiness<br />

for consultant practice. All trainees<br />

who have entered Level 3<br />

training (ST6-8) on or after 1<br />

August 2011 will be required to<br />

undertake START before applying<br />

for their CCT. The first assessments<br />

will take place on 9/10 th<br />

November <strong>2012</strong> in London having<br />

being piloted on 2 groups of trainees.<br />

START assesses many aspects<br />

of practice in 12 eight minute<br />

stations including handover,<br />

teaching, conflict/risk management<br />

and prescribing. All assessors<br />

receive training and there are<br />

2 representatives from the <strong>Peninsula</strong>.<br />

There has been excellent<br />

engagement of <strong>Peninsula</strong> consultants<br />

with the deanery educational<br />

supervisor training programme<br />

indicating a clear desire of the vast<br />

majority to remain actively involved<br />

with educational supervision.<br />

Simulation training has<br />

moved forward with a clear<br />

School strategy for workplace<br />

based training in individual Trusts.<br />

This was the basis of the successful<br />

innovation fund bid which<br />

secured funding for Training the<br />

Trainers courses. There is interest<br />

in setting up our own Stepping up<br />

to Registrar (currently purchased<br />

from Severn) and Neonatal Key<br />

competencies courses. There were<br />

3 applications to the Innovation<br />

fund this year.

School Highlight—Obstetrics & Gynaecology<br />

The School of Obstetrics and<br />

Gynaecology has undergone<br />

some changes in the last 12<br />

months.<br />

Mr Ben Peyton Jones, consultant<br />

and RCOG tutor in<br />

Exeter has been appointed<br />

the Training Programme<br />

director for ST1 and ST2<br />

doctors and will be responsible<br />

for their placements on<br />

the rotation, monitoring their<br />

progress and giving them<br />

advice on any aspects of<br />

their training where appropriate.<br />

Mr Narayanan in Torbay<br />

continues as Training<br />

Programme Director for<br />

years ST3-5 and Miss Susie<br />

Bates in Truro continues as<br />

Training Programme Director<br />

for Year ST6-7 and also<br />

is Director of ATSMs.<br />

Recent surveys have shown<br />

that all units in the <strong>Peninsula</strong><br />

have broadly satisfactory<br />

local teaching sessions for<br />

doctors at SHO level including<br />

F2, GP registrars, ST1<br />

and ST2 doctors but specific<br />

teaching for registrar grades<br />

has been lacking. The school<br />

has obtained funding to improve<br />

regional teaching for<br />

specialist registrars and a<br />

minimum of 4 training days<br />

will be provided, one by<br />

each of the main hospitals.<br />

These days will be known as<br />

“POET” days, (<strong>Peninsula</strong><br />

Obstetrics and Gynaecology<br />

Education & Training days)<br />

designed to address the<br />

needs of doctors between<br />

ST3-ST7. These days will<br />

also be made available to<br />

Severn Trainees.<br />

The main development from<br />

the RCOG is the publication<br />

of the Training Matrix which<br />

specifies the requirements<br />

for each year of training<br />

which trainees must achieve<br />

in order to get a satisfactory<br />

ARCP. All trainees were<br />

notified of this change last<br />

year and it is available on<br />

the RCOG website as well<br />

as the <strong>Deanery</strong> website. The<br />

aim is to make ARCPs all<br />

over the country comparable<br />

and improve standards.<br />

Training has to be very carefully<br />

documented these<br />

days and it is in each trainee’s<br />

best interest to study the<br />

matrix and make sure that<br />

they acquire work based<br />

assessments throughout the<br />

year. The matrix makes it<br />

easier to know exactly how<br />

many assessments are needed,<br />

what courses must be<br />

attended and what practical<br />

procedures need to be mastered<br />

by the end of each<br />

year.<br />

Raleigh Building<br />

The <strong>Peninsula</strong> school of surgery<br />

supports surgical training<br />

in all specialties except pediatric<br />

surgery.<br />

In 2011 Nick Markham stepped<br />

down as Head of School having<br />

created a strong team and<br />

reputation for the <strong>Peninsula</strong>.<br />

Ken Hosie is now the Head of<br />

School supported by Heather<br />

Rowley as School Manager.<br />

There is an experienced team<br />

of Program Directors in all<br />

Trish Boorman being appointed<br />

to Program Director there is<br />

an opportunity in Exeter for an<br />

enthusiastic tutor.<br />

Simeon Brundell continues to<br />

organize the regional core<br />

teaching program and has put<br />

together a very effective<br />

monthly teaching day around<br />

the region.<br />

All surgical training posts are<br />

appointed at national selection.<br />

Increasing competition for<br />

training numbers along with a<br />

national agenda to reduce<br />

training posts in most surgical<br />

specialties has made it essential<br />

that we provide the best<br />

available opportunities for<br />

training so that we can continue<br />

to attract the best trainees<br />

and be confident they will get<br />

the positions they want.<br />

Minimum standards for surgical<br />

training are being set nationally<br />

and monitored through<br />

the ISCP website. This provides<br />

objective evidence of the<br />

training received and who is<br />

delivering that training. That<br />

along with GMC requirements<br />

to demonstrate that trainers<br />

are trained appropriately, those<br />

who are committed and want<br />

to train will be highlighted.<br />

The School has been very<br />

effective in organizing a bespoke<br />

course for surgeons<br />

which has been very popular<br />

and achieves all the GMC requirements.<br />

This year alone 90<br />

consultants have completed<br />

the course. The School would<br />

encourage anyone who wishes<br />

to continue to train to access<br />

this course or the <strong>Deanery</strong><br />

approved courses in the near<br />

The School has been successful<br />

in acquiring a complete set<br />

of Anatomy models which can<br />

be made available for anyone<br />

wishing to refresh/ learn there<br />

anatomy (PGMC Plymouth).<br />

The School will also be keen to<br />

support anyone who wishes to<br />

develop or introduce new and<br />

innovative local training opportunities.<br />

With limited study<br />

leave budgets there is a need<br />

to provide local courses such<br />

as CCRISP and ATLS in a cost<br />

effective way.<br />

All committed and enthusiastic<br />

surgical trainers are encouraged<br />

to get involved!
