more about this disease that takes
mothers, grandmothers, daughters and
sisters from our community. As a heart
survivor and thriver, I have a vested
interest, but I’m also a proud advocate
having served a number of Go Red
campaign roles over the years in an
effort to help thousands of women live
longer, healthier lives.
You can help, too. Simply be aware
of your risks and know the facts about
this largely preventable disease. For
example, did you know, only onefourth
of all adult American women
engage in the weekly recommended
amount of physical activity (30 minutes
or more five times a week)? Or that
an estimated 62 percent of American
with over $174,000 in grants. One of
the programs, which received $35,000
of support last year, is the ElderCenter,
Parma Hospital’s full-service adult day
care center.
ElderCenter provides high-quality
services such as medication management,
meals and therapeutic recreation.
People served through the Center may
have mild to moderate Alzheimer’s Disease,
physical impairments, or they may
be socially isolated. ElderCenter also
offers the Club for members with mild
memory loss and Club SODA,
which serves active young
adults who can benefit from a
structured, supervised program.
Funding in 2008 went
toward ElderCenter’s
innovative DAY (Designed
Around You) program, which
gives caregivers valuable time
to access needed resources
while patients gain strength as
they recover from a hospital
stay and prepare to return to
independent living.
Other major grants include:
• Parma Health Ministry
residents, $43,367
• American Cancer Society,
• Renaissance scholarships
DOC AT THE DOOR
Walk right in – the ER doctor is waiting for you
Tom Basista hardly believed it was
possible. So when he needed urgent
care the night before a cross-country
flight, he hesitated. Recalling long nights
in the Emergency Department with various
family members through the last
several decades, he feared he’d get little
sleep before heading to the airport at 7
a.m.
“I thought I’d be there for hours,”
said Basista, who had never experienced
an ED visit that lasted less than several
hours. “But this was a unique experience
from start to finish.”
Doc at the Door puts a physician-led
team on the front line of care. The team
registers and assesses each patient,
usually within 30 minutes of arrival, performing
EKGs, starting IVs and ordering
tests. Parma Hospital is the first hospital
in the Cleveland area to handle ED patients
with this efficient new approach,
says Jesse DiRando, MD, chairman, Department
of Emergency Medicine.
Testing can be ordered and treatment
begun before the patient even
goes back into an ED room. Some – like
Basista – are discharged without ever
having to enter a patient bed. On a
Monday night late last summer, he was
taken care of and on his way home in
less than 30 minutes.
“The nurse came right to me in
the front of the hospital,” said Basista,
whose hand was swollen and sore after
a kitchen accident. “The wonderful part
was that the care was not in stages. Everything
was assessed and treated immediately.”
Basista received a tetanus shot,
had his antibiotic prescription filled at a
24-hour pharmacy and made his 9 a.m.
flight the next day.
“I woke up the next morning and I
felt like a new man,” said Basista, 52.
“This was the fastest experience I’d had
in an ED in 30 years. The nurse and
physician were phenomenal. They were
very cordial and compassionate.”
Hospitals from as far away as Hawaii,
Oklahoma and Connecticut have
traveled to Parma Hospital to study and
learn from Parma’s success. Introduced
nearly two years ago, the program was
expanded
recently to
be available
from noon
to 10 p.m.
everyday,
including
weekends.
The
ED recently
completed
renovation to
remove registration
areas
off the waiting room and replace them
with more private triage rooms. Since
patients are registered at the bedside,
separate registration suites are no longer
needed.
“We put the whole team at the front
door to support patient needs, making
the delivery of care quicker and more
efficient,” says Dawn Beljin RN, director
of ED operations.
The success of Doc at the Door is
evident in the Hospital’s recent patient
satisfaction scores, which are in the 90th
percentile for the past three months. In
that same period, compared to other
hospitals across the nation with more
than 40,000 patient visits per year,
Parma Hospital has averaged 30 minutes
from the time the patient walks in the
door (or arrives by ambulance) and is
evaluated by an ED doctor. The national
average is over 60 minutes.
“We beat all national benchmarks,”
says Jesse DiRando, MD,
chairman, Department of Emergency
Medicine. “These are key indicators
of patient satisfaction, department efficiency
and patient safety with risk
management.”
ED physicians like Gregory Oswald, MD, top photo, and Taras Napora, MD,
left, lead triage teams that see patients upon their arrival in the Emergency
Department.
Knowing what to expect is
half the battle.
Patients planning to have
orthopedic surgery on knees, hips or
shoulders now receive another bonus
for choosing Parma Community General
Hospital. It’s called Joint Camp.
It’s a comprehensive program that
starts at least a month before surgery
to prepare patients for the adjustments
they will need to make for a better
recovery.
“Parma Hospital already is
recognized for its five-star orthopedic
program,” says Pam Yusz, RN, director
of Surgical Services. “Joint Camp can
enhance our quality outcomes.”
The program is not mandatory, but
it is advised for all patients planning
to have joint replacement surgery.
It consists of three sessions prior to
surgery:
• An educational presentation to
review what to expect before surgery
and during the hospital stay.
• A group meeting with other
patients, led by a physical therapist.
This session will cover exercises that
will improve the patient’s recovery
and assistive devices that can help the
patient’s mobility.
• A pre-admission visit just before
surgery that involves lab and radiology
testing and a physical examination.
Joint Camp starts four to six weeks
prior to surgery. Patients are encouraged
to bring a support person, such as a
family member or friend, who can serve
as a coach during the preparation and
recovery.
Each patient in Joint Camp also
five-star orthopedics
Joint Camp team prepares
orthopedic patients for complete
recovery
benefits from the expertise of a team of
professionals that includes surgeons,
nurses, and physical and occupational
therapists.
“If you have someone learning the
exercises with you, it will expedite your
care,” says Yusz.
A primer for recovery
As a surgical nurse, Debbie Stender
has witnessed firsthand the orthopedic
surgeon stitching new joints into their
patients, making them whole and
functional again.
But even she didn’t know what to
expect after surgery when her hips and
knees were replaced.
Stender underwent
her first
hip replacement in
2002 and the second
one in 2005.
In 2007, she had
bilateral knee replacements.
She envies the
lucky patients who
can attend Joint
Camp for a primer
on post-op life. Her
husband, Rand,
would have been a
willing participant
and coach. Her
surgeon, Richard
Gittinger, MD,
believes Stender
would have been
an ideal candidate.
“I could see
how my husband
The Center for
Orthopedic Care
would have benefitted from Joint Camp
and from knowing what to expect when
I came home,” says Stender. “This camp
is fabulous. It should alleviate a lot of
patients’ apprehensions.”
Joint Camp focuses on total wellness, inviting
patients to take an active role in their hospitalization
and recovery.
To learn more about Joint Camp, talk to your
orthopedic surgeon or call Janet Babinyecz, the
clinical orthopedic nurse who coordinates the
camp, at 440-743-4428.
“Now I can swing my leg up over the motorcycle and ride without worrying about the
pain,” says Debbie Stender, who underwent bilateral knee replacement surgery in August
2007, a few years after hip replacements. Her husband, Rand, helped her recover.
GERIATRIC HIP FRACTURE PROGRAM
Prompt surgery helps patients recover faster
Hip fractures are a common problem
for older Americans, with hundreds
of thousands suffering these injuries
each year.
The National
Center for Injury Prevention
and Control
predicts a doubling
of the senior population
between 2000
and 2020. The growth
of this population
Richard Gittinger, MD
likely results in a
considerable increase in hip fractures,
which have a mortality rate of at least
30 percent. Of those who return home
after treatment, only 20 percent have the
same function as they did before suffering
a fracture.
The new Geriatric Fracture Program
at Parma Hospital is a team approach
that initiates protocols as soon as the
patient arrives at the hospital. Most patients
require surgery, and the goal is to
better prepare them for surgery and get
them to the operating room within 24 to
36 hours of admission.
“The faster you get patients into
surgery, the better they do overall,” says
Pam Yusz, director of Surgical Services.
The basic principle of treating hip
fractures is to secure the fracture, promoting
healing and early mobilization.
Prompt surgical care and close postoperative
care is essential to better outcomes,
reducing pneumonia, delirium,
skin breakdowns, and a return to prefracture
mobility.
The team, including orthopedic
surgeons, geriatricians and specialized
nurses, review the patient’s Vitamin
D levels and nutritional and osteoporosis
status. Patients undergo a bone
density screening within a few weeks
after surgery to determine if they need
osteoporotic drugs to prevent future
fractures, which is the goal of the new
program.
“Parma Hospital has always done
a great job taking care of hip fracture
patients, and much of this program was
already in place,” says Richard Gittinger,
MD, an orthopedic surgeon who helped
structure the program. “We are just refining
the process to make sure all patients
receive optimal care.”
Even patients just considering a joint replacement, but not yet scheduled, can attend Joint Camp. John
Newton, right, of Independence, was considering surgery at another hospital when he learned of Parma
Hospital’s Joint Camp. Already pleased with his choice of orthopedic surgeon, Blane McCoy, MD, he attended
an introductory Joint Camp meeting and began his journey toward wellness.
Cindy Fousek, physical therapy assistant, Courtney Hanak, patient care assistant, and Jody Loughner, RN,
worked with Newton during his hospital stay.
HealthGrades:
Outcomes exceed
national averages
Parma Hospital has received fivestar
ratings from HealthGrades, an
independent
healthcare ratings
company,
in three areas of
orthopedics:
• joint
replacement
• total knee
replacement
• hip fracture repair
HealthGrades found that patients
treated at five-star rated hospitals for
nine common procedures are 47 percent
less likely to experience a major
complication, such as post-operative
infections or heart conditions that can
lengthen hospital stays and increase
costs. According to the study, if all
Medicare patients had been treated at
a top-performing hospital compared to
the lowest-performing hospitals during
2005-07, more than 123,000 major
complications potentially could have
been avoided.
Parma Hospital participates in the
Surgical Care Improvement Project
(SCIP), developed by Joint Commission
and the Centers for Medicare and Medicaid
Services as part of Core Measures,
which identify best practices and outcomes
in patient care. Parma Hospital’s
orthopedic surgeons exceed that of the
national average in ten related measures,
based on the Midas+ National
Comparative Data Base for the first quarter
of 2008.
“Parma Hospital’s consistent recognition
from HealthGrades for outcomes
that exceed national averages
are proof that providing personalized,
specialized care matters,” said Parma
Hospital President & CEO Patricia A.
Ruflin. “The orthopedic physicians,
nursing, and pharmacy staff at Parma
Hospital have repeatedly demonstrated
their commitment to improved standards
and the best outcomes for their
patients.”
