Forecast: Sunny days, blue skies after new hip<br />
Well-known weatherman<br />
Brad Sussman looked forward to<br />
his hip replacement with sheer<br />
enthusiasm, knowing he faced a<br />
faster recovery than he had following<br />
a joint replacement nine<br />
years ago.<br />
Seeking the least invasive option<br />
possible, Sussman chose Dr.<br />
Larry Lika of Southwest Orthopaedics<br />
for the anterior approach to<br />
minimally invasive hip replacement.<br />
“I was blown away by the difference,”<br />
said Sussman, whose<br />
first hip replacement nearly a decade<br />
ago had been state-of-theart<br />
at the time. “This is absolutely<br />
the way to go.”<br />
In 20 years of practice, Dr.<br />
Lika has had many patients who<br />
have experienced the posterior<br />
approach on one hip and the<br />
anterior approach on the other<br />
side. With the anterior approach,<br />
the surgeon reaches the hip from<br />
the front of the joint between<br />
two muscles, rather than cutting<br />

through muscle and reattaching<br />
it. The hip ball and socket are<br />
removed and replaced, through<br />
small incisions, limiting post-operative<br />
discomfort.<br />
“Minimally invasive doesn’t just<br />
mean a smaller incision,” says Dr.<br />
Lika. “It also means that you’re<br />
not destroying all the muscle and<br />
tissue below it, which is where the<br />
pain comes in.”<br />
Sussman was standing and<br />
walking the day after surgery and<br />
went home from <strong>Parma</strong> <strong>Hospital</strong><br />
three days later.<br />
“There is a day and night difference,”<br />
says Dr. Lika, whose<br />
patients come to the office two<br />
weeks after surgery without a<br />
cane or a walker.<br />

Therapy strengthens new hips<br />
Less than three weeks after surgery to replace<br />
his hip, Brad Sussman was cycling on a<br />
stationary bike and doing squats and side leg<br />
raises.<br />
While these exercises took place under the<br />
watchful eye of physical therapist Mike Scharf,<br />
they would have been difficult to even imagine<br />
doing so soon after his first hip replacement, a<br />
traditional surgery over nine years earlier.<br />

crossing their legs or ankles. Such motions<br />
could dislocate the new hip.<br />
“With the anterior approach, the<br />
therapists can be much more aggressive<br />
in rehab, because you don’t<br />
have to worry about dislocation,” says<br />
orthopedic surgeon Larry Lika, DO.<br />
“The muscles have not been disturbed.”<br />
“You will never fully get your function<br />
back without physical therapy,”<br />
Scharf said. “Patients really don’t<br />
know where to start on their own,<br />
or they can develop other problems<br />
by performing exercises the wrong<br />
way.<br />
“Our brain is smart and finds<br />
ways to compensate and avoid<br />
pain. Therapists are trained to help<br />
patients work through that and<br />
exercise safely.”<br />

The changes in Shirley Harnest’s<br />
89-year-old father were subtle. But<br />
his inexplicable mental confusion<br />
and suddenly shaky health status<br />
were enough for an attentive daughter<br />
to notice.<br />
Cases like Bill Masek’s are the<br />
specialty of the Geriatric Assessment<br />
program at <strong>Parma</strong> <strong>Hospital</strong>. A<br />
physician-led team of professionals<br />
provides a thorough evaluation of<br />
older patients to establish a baseline<br />
of daily functionality. Then they refer<br />
families to support services and appropriate<br />
medical care.<br />
“He would have moments of being<br />
really clear and then lapse into<br />
this fog,” said Harnest. “He wasn’t<br />
eating properly or taking his pills like<br />
he should. I wanted to get him evaluated<br />
by a geriatric specialist to see if<br />
there were underlying problems.”<br />
A detailed assessment covers the<br />
patient’s physical, mental and social<br />
wellness. Blood work and other tests<br />
may be ordered. The geriatric team,<br />
including the doctor, nurse and<br />
social worker, then meets with the<br />
family to discuss their findings.<br />

In Masek’s case, a medication he<br />
was taking contributed to his compromised<br />
state. He is now under the<br />
care of Dr. Gorjanc, an internal medicine<br />
doctor with a focus on geriatrics.<br />
He passed his driving evaluation<br />
and has returned to his <strong>Parma</strong> home,<br />
aided by a medical alert system that<br />
will notify his family and a response<br />
team in case of emergency.<br />

unit from surgery through<br />

What is special about robotic surgery?<br />
<strong>Parma</strong> <strong>Hospital</strong> has the latest version of<br />
the surgical platform for robotic surgery.<br />
Robotic surgery is a tool to provide the<br />
best minimally invasive options that translate<br />
into shorter hospital stays, less pain,<br />
less blood loss and lower risk of infection.<br />
What types of gynecologic surgeries can<br />
be performed?<br />
Most procedures, particularly hysterectomies<br />
(removal of the uterus), oophorectomies<br />
(removal of ovaries) and myomectomies<br />
(surgical removal of fibroid tumors<br />
that leaves the uterus intact), are now<br />
being performed robotically. We can also<br />
perform robotic surgery for endometriosis,<br />
pelvic organ prolapse and tubal ligation.<br />
Remember, the surgeon is still doing<br />
the surgery, but they operate by controlling<br />
the robot.<br />
Does it improve my recovery in any way?<br />
Recovery is quite a bit faster for patients<br />
who choose this minimally invasive option.<br />
The incisions are tiny and less tissue<br />
is disrupted, making the healing process<br />
smoother. All of my da Vinci patients have<br />
successfully been discharged without<br />
needing any narcotic pain medication.<br />
Patients leave the hospital sooner, which<br />
translates into a quicker return to normal<br />
activities.<br />
Are there cases when robotic surgery is<br />
not advisable?<br />
Cases that involve severe pelvic adhesions<br />
or particularly large tumors are not<br />
recommended to be done robotically. Talk<br />
to your doctor about your specific case.<br />
