packet. Either make photocopies from this packet, or download an<br />

4) A completed Portfolio also includes a cover page (use the templates provided in<br />
your Teacher Workshop packet).<br />
5) Compile your completed Portfolio in a three ring binder*, placing documents in<br />
the following order. Remember that each <strong>Class</strong>room or Grade Level should<br />
submit their own Portfolio.<br />
*You may also submit a paperless (digital) portfolio, provided<br />
you are able to provide the necessary documentation (i.e.<br />
photographs, recordings, examples of student work)<br />

Documentation for <strong>Act</strong>ivity (student work or footage/photos)<br />

6) Please give your completed Portfolio to your Teacher Representative by June<br />
3, <strong>2013</strong>. Teacher Representatives should turn all Portfolios in to their<br />
Regional Manager by June7, <strong>2013</strong> and should ensure that they have a<br />
completed Portfolio from each classroom or grade level.<br />
Helpful Tips<br />
‣ In choosing which activities to undertake with your students, focus on<br />
selecting activities that allow you to address a variety of developmental<br />
areas<br />
‣ All adults involved in the Portfolio process are responsible for its<br />
completion. Aids and resource specialists should be encouraged to<br />
include their notes and observations<br />
‣ Allow the students to be a part of the process, to select which work will<br />
be included in the Portfolio, and to participate in the assessment<br />
process<br />
