APPENDIX 3-B<br />

REQUEST FOR SUPERVISORY CANDIDATE STATUS<br />

The report of this meeting includes:<br />

A. a record that the applicant provided documentation of the formal requirements:<br />

• photocopies of accredited college <strong>and</strong> seminary diplomas or transcripts or<br />

documentation of M.Div. degree equivalency.<br />

• documentation of ACPE clinical membership <strong>and</strong> Accountability for Ethical Conduct<br />

<strong>Policy</strong> Report form.<br />

• letter(s) from faith group official(s) documenting both the applicant’s faith group<br />

endorsement <strong>and</strong> "good st<strong>and</strong>ing" on rolls of faith group. (See Appendix 5<br />

ACPE Ecclesiastical Endorsement Conflict Resolution <strong>Policy</strong>.)<br />

• ordination or commission to function in ministry by an appropriate religious authority.<br />

• completion of at least one unit of Supervisory CPE.<br />

• see Appendix 7 for instructions on entering the certification process.<br />

B. descriptions written under the headings of:<br />

• Evaluation of materials presented.<br />

• Summary of interview (the course of the interview, how the applicant met the<br />

committee, how the committee engaged the applicant.)<br />

• Brief evaluation of:<br />

(1) The applicant's underst<strong>and</strong>ing of the applicant’s personal/cultural history <strong>and</strong><br />

strengths <strong>and</strong> weaknesses as a potential pastoral educator.<br />

(2) The applicant's capacity to articulate a theological underst<strong>and</strong>ing of life from within<br />

the applicant’s tradition.<br />

(3) The applicant's demonstrated pastoral competence.<br />

(4) The applicant's demonstrated conceptual competence.<br />

(5) The applicant's underst<strong>and</strong>ing of the applicant’s desire to enter the certification<br />

process <strong>and</strong> of the applicant’s potential as a CPE supervisor.<br />

• Strengths <strong>and</strong> areas of needed growth.<br />

• Committee suggestions <strong>and</strong>/or recommendations.<br />

C. Clear indication whether request for status granted or denied<br />

D. Names of committee members.<br />
