NUMBER AND DISTRIBUTION<br />

OF SUB-DIVISIONAL MANAGERS<br />

The Census <strong>of</strong> Population <strong>and</strong> Hous<strong>in</strong>g divides Fiji <strong>in</strong>to four<br />

divisions <strong>and</strong> 15 prov<strong>in</strong>ces as shown <strong>in</strong> Table 1. The MoH<br />

is managed from its headquarters <strong>of</strong>fice <strong>in</strong> Suva, through<br />

the four divisions <strong>and</strong> 19 sub-divisions, 16 <strong>of</strong> which have<br />

a sub-divisional hospital. The three urban sub-divisions <strong>of</strong><br />

Suva (the capital), Labasa <strong>and</strong> Lautoka/Yasawa are with<strong>in</strong> the<br />

catchment areas <strong>of</strong> the three divisional hospitals, however,<br />

they manage <strong>and</strong> provide the full range <strong>of</strong> community <strong>health</strong><br />

services through <strong>health</strong> centres <strong>and</strong> nurs<strong>in</strong>g stations with<strong>in</strong><br />

both urban <strong>and</strong> peri-urban areas.<br />

Good perform<strong>in</strong>g <strong>health</strong> adm<strong>in</strong>istrators at the<br />

divisional <strong>and</strong> sub-divisional levels may be<br />

transferred to the MoH headquarters or to<br />

other m<strong>in</strong>istries, such as Foreign Affairs.<br />

For the purposes <strong>of</strong> this <strong>review</strong>, the <strong>management</strong> situation<br />

fac<strong>in</strong>g the 19 SDMOs <strong>and</strong> their adm<strong>in</strong>istrative support<br />

staff are considered. The SDMOs <strong>and</strong> adm<strong>in</strong>istrators are<br />

responsible for manag<strong>in</strong>g an <strong>in</strong>tegrated <strong>health</strong> service with<strong>in</strong><br />

the sub-division; manag<strong>in</strong>g the hospital <strong>and</strong> <strong>health</strong> centres,<br />

coord<strong>in</strong>at<strong>in</strong>g public <strong>health</strong> services delivered by public<br />

<strong>health</strong> nurses, <strong>health</strong> <strong>in</strong>spectors, dentists, nutritionists <strong>and</strong><br />

other <strong>health</strong> cadres <strong>in</strong> the sub-divisional establishment<br />

(Government <strong>of</strong> Fiji 2006a).<br />

Adm<strong>in</strong>istrative staff are based <strong>in</strong> the sub-divisional <strong>of</strong>fices,<br />

usually at the hospital, <strong>and</strong> conduct general adm<strong>in</strong>istration,<br />

oversee budget processes, procurement, travel, <strong>in</strong>formation<br />

systems <strong>and</strong> manage the general staff. These adm<strong>in</strong>istrative<br />

positions are common cadre positions filled largely by<br />

employees <strong>of</strong> the PSC. Good perform<strong>in</strong>g <strong>health</strong> adm<strong>in</strong>istrators<br />

at the divisional <strong>and</strong> sub-divisional levels may be transferred<br />

to the MoH headquarters or to other m<strong>in</strong>istries, such as<br />

Foreign Affairs (In-country <strong>review</strong>ers 2010).<br />

In 2009, <strong>in</strong> response to fiscal pressures the PSC reduced the<br />

retirement age from 60 years to 55 (Amosa et al. 2009). The<br />

MoH lost about 331 experienced senior personnel, <strong>in</strong>clud<strong>in</strong>g<br />

15 doctors <strong>and</strong> 97 nurses <strong>and</strong> support staff (Sharma 2009).<br />

A number <strong>of</strong> strategies to deal with the sudden loss <strong>of</strong> senior<br />

staff with corporate memory are <strong>in</strong> place, such as re-engag<strong>in</strong>g<br />

retired staff on contracts, undertak<strong>in</strong>g <strong>in</strong>ternal redeployments<br />

<strong>and</strong> out-sourc<strong>in</strong>g certa<strong>in</strong> services (Sharma 2009).<br />
