conducts monthly or bi-monthly meet<strong>in</strong>gs with all CHW Supervisors, aga<strong>in</strong>, <strong>for</strong> report<strong>in</strong>g,<br />

plann<strong>in</strong>g <strong>and</strong> cont<strong>in</strong>uous refresher tra<strong>in</strong><strong>in</strong>g (Csaey 2005).<br />

Supervision of voluntary cadres is often weak <strong>and</strong> vulnerable to changes caused by re<strong>for</strong>m. A<br />

study <strong>in</strong> Indonesia revealed that after decentralisation family plann<strong>in</strong>g field staff chose to<br />

move to other government positions with better status <strong>and</strong> payment. This left village family<br />

plann<strong>in</strong>g volunteers with no supervisors or on-go<strong>in</strong>g tra<strong>in</strong><strong>in</strong>g. (Utomo, Arsyad et al. 2006)<br />

The supervision of these cadres should be considered <strong>in</strong> any re<strong>for</strong>m which may be assisted by<br />

clear, available <strong>and</strong> up to date <strong>in</strong><strong>for</strong>mation of the supervisory structures at this level. A FHI<br />

st<strong>and</strong>ards toolkit provides some guidance <strong>for</strong> the supervision of PHEs. It calls <strong>for</strong> supervisor<br />

tra<strong>in</strong><strong>in</strong>g <strong>and</strong> the need <strong>for</strong> supervisors to ensure that PHEs have received adequate preparation<br />

(through tra<strong>in</strong><strong>in</strong>g <strong>and</strong> skills acquisition/practice), that motivation <strong>and</strong> ethical behaviour is<br />

cont<strong>in</strong>ually re<strong>in</strong><strong>for</strong>ced, group dynamics are managed <strong>and</strong> team build<strong>in</strong>g is encouraged <strong>and</strong><br />

that responsibility is shared with PHEs (Family <strong>Health</strong> International 2005).<br />

The supervision of TBAs can vary <strong>and</strong> depends on the amount of government support they<br />

receive. Various models <strong>in</strong>clude supervision can be undertaken by the community such as <strong>in</strong><br />

some African countries where the TBA participates <strong>in</strong> a village health committee. In the<br />

Philipp<strong>in</strong>es some well per<strong>for</strong>m<strong>in</strong>g TBAs have been tra<strong>in</strong>ed as supervisory staff, while <strong>in</strong><br />

Brazil <strong>and</strong> to an extent <strong>in</strong> Senegal, literate TBAs staff maternity centres (Walt 1986).<br />

Educated health professionals normally per<strong>for</strong>m a supervisory role <strong>in</strong> the <strong>for</strong>mal health<br />

sector. Examples are the supervision of village dais by auxiliary nurse midwives <strong>in</strong> the<br />

Varanasi District of Uttar Pradesh, India (Intra<strong>Health</strong> 2002).<br />

Supervision <strong>and</strong> health worker per<strong>for</strong>mance<br />

Accord<strong>in</strong>g to the Cochrane database a systematic study is currently be<strong>in</strong>g undertaken to<br />

assess the impact that supervision outreach visits have on the quality of primary health care <strong>in</strong><br />

low- <strong>and</strong> middle-<strong>in</strong>come countries. However no results are available as yet (Bosch-Capblanch<br />

<strong>and</strong> Garner 2009). Despite this the f<strong>in</strong>d<strong>in</strong>gs of various studies <strong>and</strong> evaluations suggest that<br />

health worker per<strong>for</strong>mance is improved when supervision is l<strong>in</strong>ked to a range of supportive<br />

activities. A review of issues relat<strong>in</strong>g to the per<strong>for</strong>mance of community-level health <strong>and</strong><br />

nutrition functionaries <strong>in</strong> India found that the quality of their work improved when<br />

supervision is:<br />
