The PDM is an adaptation <strong>and</strong> ref<strong>in</strong>ement of the CPDMS tool that was trialled <strong>in</strong> three<br />

community health centres <strong>in</strong> Indonesia. The focus is on 4 <strong>in</strong>struments to measure cl<strong>in</strong>ical<br />

skills. The specific context is not MNRH although it could be applied <strong>in</strong> this sett<strong>in</strong>g.<br />

However the tool does not assess health education, promotion <strong>and</strong> <strong>in</strong><strong>for</strong>mation activities of<br />

the health worker that are a key part of MNRH. The evaluation found that successful<br />

implementation of this approach depends on a supportive system from district health offices,<br />

supportive district government policy <strong>and</strong> commitment from managerial <strong>and</strong> cl<strong>in</strong>ical staff.<br />

This evaluation, although undertaken <strong>in</strong> a rapid time frame shows that the PDM <strong>in</strong>strument<br />

development has the potential to improve the cl<strong>in</strong>ical per<strong>for</strong>mance of all the cl<strong>in</strong>ical staff <strong>in</strong><br />

community health centre however further research is necessary. It could be adapted to<br />

MNRH contexts to <strong>in</strong>clude health promotive skills <strong>and</strong> extended to community outreach<br />

sett<strong>in</strong>gs.<br />

At the request of the USAID MCH team, the <strong>Health</strong> Care Improvement (HCI) Project<br />

developed a tool that def<strong>in</strong>es a set of key elements that are needed <strong>for</strong> community health<br />

worker programs to function effectively <strong>and</strong> measures how well programs meet these criteria.<br />

The tool is applied <strong>in</strong> an easy-to-implement assessment process that provides rich<br />

opportunities to identify ways to further develop programs to enhance their effectiveness <strong>and</strong><br />

impact on maternal <strong>and</strong> child health goals. The tool exam<strong>in</strong>es 12 programmatic components<br />

that CHW programs should consider as important to successfully support<strong>in</strong>g CHWs. These<br />

<strong>in</strong>clude: recruitment process; the CHW role; <strong>in</strong>itial tra<strong>in</strong><strong>in</strong>g; ongo<strong>in</strong>g tra<strong>in</strong><strong>in</strong>g; equipment <strong>and</strong><br />

supplies; supervision; per<strong>for</strong>mance evaluation; <strong>in</strong>centives; community <strong>in</strong>volvement; referral<br />

system; professional advancement; <strong>and</strong> documentation/<strong>in</strong><strong>for</strong>mation management. In apply<strong>in</strong>g<br />

the tool, each component is rated with a four-po<strong>in</strong>t scale rang<strong>in</strong>g from non-functional to<br />

highly functional. In addition to be<strong>in</strong>g part of functional systems, CHWs must be provid<strong>in</strong>g<br />

services <strong>in</strong> MCH. A list of <strong>in</strong>terventions, adapted <strong>for</strong> the CHW role from the key MCH<br />

<strong>in</strong>terventions listed <strong>in</strong> USAID‘s 2008 Report to Congress is also <strong>in</strong>cluded <strong>in</strong> the tool. This<br />

result<strong>in</strong>g <strong>in</strong>strument can be applied <strong>in</strong> a stakeholder meet<strong>in</strong>g to assess the current status of a<br />

specific program <strong>and</strong> determ<strong>in</strong>e if the program as a whole is functional. <strong>Health</strong> workers<br />

with<strong>in</strong> that program are then considered to be functional. Evaluations of this tool <strong>in</strong> LMICs<br />

could not be located <strong>and</strong> as a result its merit is unknown.<br />
