Executive Summary<br />
Address<strong>in</strong>g the barriers <strong>and</strong> constra<strong>in</strong>ts faced by health workers <strong>and</strong> managers is a key<br />
component <strong>in</strong> health system strengthen<strong>in</strong>g <strong>and</strong> improv<strong>in</strong>g the quality of health care <strong>in</strong> low<br />
<strong>and</strong> low middle <strong>in</strong>come countries <strong>in</strong> the Asia <strong>and</strong> Pacific regions. A number of reports (de<br />
Savigny 2009; WHO 2009 ) have highlighted the lack of knowledge available to policy<br />
makers <strong>and</strong> practitioners concern<strong>in</strong>g tools <strong>and</strong> approaches that have proved useful <strong>in</strong><br />
address<strong>in</strong>g <strong>HRH</strong> challenges <strong>in</strong> similar contexts. Such knowledge serves to support <strong>in</strong><strong>for</strong>med<br />
decision mak<strong>in</strong>g <strong>and</strong> help countries prioritize the limited resources available <strong>for</strong> systems<br />
strengthen<strong>in</strong>g. There is there<strong>for</strong>e a need to underst<strong>and</strong> what tools work <strong>in</strong> specific sett<strong>in</strong>gs<br />
<strong>and</strong> to identify lessons from successful (<strong>and</strong> unsuccessful) re<strong>for</strong>m ef<strong>for</strong>ts that can be applied<br />
by countries with comparable objectives. However, assess<strong>in</strong>g what constitutes a successful<br />
<strong>HRH</strong> <strong>in</strong>tervention <strong>and</strong> the extent to which it translates <strong>in</strong>to enhanced management, team <strong>and</strong><br />
<strong>in</strong>dividual health worker per<strong>for</strong>mance that impacts upon health outcomes at community level<br />
depends on coord<strong>in</strong>ated approaches <strong>and</strong> efficient monitor<strong>in</strong>g <strong>and</strong> evaluation systems.<br />
Knowledge of successful <strong>HRH</strong> practice <strong>in</strong> maternal, neonatal <strong>and</strong> reproductive health<br />
(MNRH) at community level is critical. Millennium development goal 5 (MDG 5) is the goal<br />
towards which least progress has been made. <strong>Maternal</strong> mortality rema<strong>in</strong>s unacceptably high<br />
<strong>in</strong> many develop<strong>in</strong>g countries, with 61% of women deliver<strong>in</strong>g alone or with an unskilled<br />
attendant <strong>and</strong> access to reproductive health services <strong>in</strong>clud<strong>in</strong>g family plann<strong>in</strong>g rema<strong>in</strong>s<br />
limited.(UNDESA 2009). The community is often the first po<strong>in</strong>t of contact women <strong>and</strong> their<br />
families have with the health system <strong>and</strong> it is at the household level that the activities of the<br />
health sector are ultimately directed (Wagstaff 2004). People centred health care is a key<br />
pr<strong>in</strong>ciple of primary health care (PHC) <strong>and</strong> health workers <strong>and</strong> human resources <strong>for</strong> health<br />
(<strong>HRH</strong>) management processes have an important role <strong>in</strong> ―enabl<strong>in</strong>g people to <strong>in</strong>crease control<br />
over, <strong>and</strong> to improve, their health‖ (WHO 1986). The community level has received renewed<br />
attention due to the revitalisation of PHC. Primary health care re<strong>for</strong>m has highlighted the<br />
need to better l<strong>in</strong>k community level care with district level services (WHO 2008) improv<strong>in</strong>g<br />
the support of <strong>HRH</strong> <strong>and</strong> strengthen<strong>in</strong>g referral mechanisms.<br />
This literature review provides examples of lessons learned <strong>in</strong> the plann<strong>in</strong>g, implementation<br />
<strong>and</strong> evaluation of <strong>HRH</strong> <strong>in</strong>terventions <strong>in</strong> MNRH at the community level <strong>in</strong> the Asia <strong>and</strong><br />
Pacific regions. The review outl<strong>in</strong>es <strong>in</strong>terventions <strong>in</strong> the areas of <strong>HRH</strong> policy, management<br />
<strong>and</strong> education <strong>and</strong> tra<strong>in</strong><strong>in</strong>g. It synthesises what are considered effective ways of work<strong>in</strong>g with<br />
the community <strong>and</strong> ways towards build<strong>in</strong>g supportive environments <strong>for</strong> health workers.<br />
Important <strong>HRH</strong> experiences from countries that have made progress towards MDG 5 are<br />
discussed as well as country experiences of scal<strong>in</strong>g up <strong>HRH</strong> <strong>in</strong>terventions <strong>in</strong> MNRH <strong>and</strong><br />
proposed models practice. F<strong>in</strong>ally approaches to the assessment of <strong>in</strong>dividual health worker,<br />
team <strong>and</strong> <strong>HRH</strong> management per<strong>for</strong>mance are presented <strong>in</strong> the context of health system<br />
strengthen<strong>in</strong>g <strong>and</strong> the achievements of Millennium Development Goal 5 (MDG 5).<br />
