Operative Technique<br />
When reaming is performed, the<br />
entire femoral canal should be overreamed<br />
down through the isthmus, in<br />
order to avoid stress riser in the bone.<br />
In order to accommodate the<br />
proximal part of the Gamma3 Nail,<br />
the subtrochanteric region must be<br />
opened up to 15.5mm (Fig. 28). This<br />
can be done either by reaming with<br />
the <strong>Stryker</strong> Bixcut Reaming System<br />
(Fig. 26) or, alternatively, with the One<br />
Step Conical Reamer. For soft tissue<br />
protection, the Conical Reamer Sleeve<br />
should be used during reaming.<br />

Warning:<br />
Care must be taken with flexible<br />
reamers to ensure that the Guide<br />
Wire is not displaced laterally<br />
during reaming. This could lead<br />
to resection of more bone on the<br />
lateral side, which in turn would<br />
lead to an offset position for the<br />
nail and a risk of shaft fracture.<br />
