test <strong>and</strong> inspection documentation. Such documentation should include credentials <strong>of</strong> special<br />

inspectors, collection <strong>of</strong> OSHPD “Test <strong>and</strong> Special Inspection” <strong>for</strong>ms, <strong>and</strong> distribution <strong>of</strong> OSHPD<br />

Staff field reports to the Owner, Contractor, <strong>and</strong> the <strong>Design</strong> Pr<strong>of</strong>essional <strong>of</strong> Record. The IOR is<br />

responsible <strong>for</strong> validating <strong>and</strong> reporting on the quality <strong>of</strong> the work done by the contractor. He or<br />

she is further responsible to submit “Verified Compliance” reports to Pr<strong>of</strong>essional <strong>of</strong> Record at the<br />

predefined progress “Milestones” as prescribed on the OSHPD approved TIO Program <strong>and</strong> he or<br />

she is responsible to keeping records relative to the status <strong>of</strong> “sign <strong>of</strong>f” by those responsible to<br />

per<strong>for</strong>m, verify, <strong>and</strong> accept the prescribed tests <strong>and</strong> inspections during construction. As out lined<br />

in the TIO the IOR will per<strong>for</strong>m all the inspections <strong>and</strong> testing as identified on the TIO which<br />

identify the IOR as the responsible party <strong>for</strong> the designated inspections.<br />

Contractors<br />

The contractor is responsible <strong>for</strong> control <strong>of</strong> construction means, methods, techniques, sequences<br />

<strong>and</strong> procedures, <strong>for</strong> providing a safe place to work, <strong>for</strong> constructing the project in accordance with<br />

the OSHPD approved <strong>Construction</strong> Documents; <strong>and</strong> <strong>for</strong> controlling the quality <strong>of</strong> construction.<br />

The contractor is also responsible <strong>for</strong> providing safe access to those elements that require tests<br />

<strong>and</strong> inspections by inspectors, OSHPD staff, design pr<strong>of</strong>essionals, <strong>and</strong> the owner. He or she is<br />

further responsible <strong>for</strong> submitting verified compliance reports to the <strong>Design</strong> Pr<strong>of</strong>essional <strong>of</strong><br />

Record at the predefined progress milestones as prescribed on the OSHPD-approved TIO<br />

Program.<br />

Special Inspectors<br />

The Special Inspectors are generally employed by the Testing Laboratory. They have<br />

qualifications <strong>and</strong> certifications that demonstrate experience with particular types <strong>of</strong> construction.<br />

They must be explicitly identified in the TIO Program <strong>and</strong> are responsible <strong>for</strong> special inspections<br />

<strong>and</strong> related documentation as prescribed in the OSHPD approved TIO Program.<br />

<strong>Design</strong> Pr<strong>of</strong>essional<br />

The design pr<strong>of</strong>essionals are responsible to identify <strong>and</strong> coordinate with the <strong>Design</strong> Pr<strong>of</strong>essional<br />

<strong>of</strong> Record in general responsible charge <strong>for</strong> all <strong>of</strong> the necessary testing <strong>and</strong> inspections within<br />

their respective design discipline. They are further responsible <strong>for</strong> conducting field visits to<br />

observe <strong>and</strong> report on conditions relative to their design specifications. Each design pr<strong>of</strong>essional<br />

is responsible <strong>for</strong> submitting a “Verified Compliance” report to Pr<strong>of</strong>essional <strong>of</strong> Record at the<br />

predefined progress “milestones” <strong>and</strong> / or intervals prescribed on the OSHPD approved TIO<br />

Program.<br />

Tests<br />

The TIO Program should clearly identify the required building elements <strong>and</strong> systems to be tested<br />

<strong>for</strong> the entire project scope. Each test should be properly named <strong>and</strong> referenced to the pertinent<br />

code <strong>and</strong> / or guideline. It should present a brief description <strong>of</strong> the test requirements <strong>and</strong> the<br />

discipline <strong>of</strong> construction to which it applies. The TIO Program should identify the progress status<br />

<strong>and</strong> persons responsible to per<strong>for</strong>m the tests, verification that the tests were conducted in<br />

con<strong>for</strong>mance with building code st<strong>and</strong>ards, <strong>and</strong> the acceptability <strong>of</strong> the tests results.<br />
