TEST, INSPECTION, AND OBSERVATION PROGRAM (TIO)<br />

INSTRUCTIONS:<br />

• The TIO Program is a part <strong>of</strong> the OSHPD approved construction documents.<br />

• The “Pr<strong>of</strong>essional <strong>of</strong> Record” (POR) is responsible <strong>for</strong> the development <strong>and</strong> administration <strong>of</strong> a project specific TIO<br />

program.<br />

• The “Pr<strong>of</strong>essional <strong>of</strong> Record” shall coordinate with all design pr<strong>of</strong>essionals assigned to the project to establish the<br />

scope <strong>of</strong> the testing <strong>and</strong> inspections.<br />

• The “Pr<strong>of</strong>essional <strong>of</strong> Record” in coordination with the “Inspector <strong>of</strong> Record” (IOR) shall be responsible <strong>for</strong> the<br />

distribution <strong>and</strong> gathering <strong>of</strong> all required “Verified Compliance Reports” <strong>and</strong> “Test <strong>and</strong> Special Inspection Forms”.<br />

• A copy <strong>of</strong> the approved TIO <strong>for</strong>m will be distributed when the building permit <strong>and</strong> IOR application have been<br />

approved by OSHPD.<br />

• A copy should be kept with the approved plans at the job site throughout construction.<br />

• A copy <strong>of</strong> the TIO program shall be maintained at the project site by the Inspector <strong>of</strong> Record (IOR) <strong>and</strong> serve as a<br />

“job card” throughout the course <strong>of</strong> construction.<br />

• Certifications <strong>of</strong> all special inspectors are required to be collected by the IOR.<br />

• They will be reviewed <strong>and</strong> approved by the IOR <strong>and</strong> OSHPD field staff <strong>and</strong> will be <strong>for</strong>warded to the OSHPD Office<br />

by the IOR in accordance with Title 24, Part 1, Section 7-155(d) <strong>and</strong> Section 7-145(a).<br />

• All design pr<strong>of</strong>essionals, the general contractor <strong>and</strong> the project inspector <strong>of</strong> record must submit Verified Compliance<br />

Reports (VCR’s) at the progress milestones (or intervals) identified in this TIO program.<br />

• When the field conditions disclose the need <strong>for</strong> additional tests, special inspections, <strong>and</strong> / or observations, the TIO<br />

program may be amended by the “Pr<strong>of</strong>essional <strong>of</strong> Record”.<br />

• The new amended TIO must be submitted to the <strong>of</strong>fice if there are “Material Alterations” to the approved plans,<br />

specifications, <strong>and</strong> /or code required tests or inspections.<br />

• When an “Amended <strong>Construction</strong> Document” revises the scope <strong>of</strong> a project <strong>and</strong> additional tests are required, a<br />

revised TIO program will be submitted to OSHPD<br />

• After the tests <strong>and</strong> inspections identified on the Approved TIO <strong>for</strong>m are completed, verified <strong>and</strong> accepted, it shall be<br />

submitted to “Pr<strong>of</strong>essional <strong>of</strong> Record” <strong>and</strong> will become part <strong>of</strong> the required project closure documentation.<br />

Responsible Personnel<br />

Successful projects include a full complement <strong>of</strong> required personnel whom are fully engaged in<br />

the process <strong>and</strong> communicate <strong>of</strong>ten <strong>and</strong> deliberately regarding the various TIO issues. Included<br />

within this group are Owners, <strong>Design</strong> Pr<strong>of</strong>essionals, Inspectors, OSHPD Staff, <strong>and</strong> Contractors.<br />

Each <strong>of</strong> these participants <strong>and</strong> their contact in<strong>for</strong>mation should be clearly presented in a Directory<br />

on the TIO Program. A Directory is optional. Although the <strong>for</strong>ms OSH-FD-303A <strong>and</strong><br />

OSH-FD-303S1 that are posted to the OSHPD Website are acceptable instruments to use <strong>for</strong> the<br />

TIO Program development, they do not include a Directory <strong>of</strong> TIO participants.<br />

Relative to the TIO Program, each must be responsible <strong>for</strong> certain aspects <strong>of</strong> the quality<br />

assurance <strong>and</strong> controls as follows:<br />
