Pr<strong>of</strong>essional in responsible charge. This in<strong>for</strong>mation should match that provided in the Application<br />

<strong>for</strong> Review.<br />

<strong>Project</strong> In<strong>for</strong>mation<br />

Provide a general description <strong>of</strong> the work (e.g., new, renovation, or addition) as applicable <strong>for</strong> the<br />

project. Use the Remodel 2010 Code Application Notice (CAN) <strong>for</strong> projects that include additions,<br />

alterations, or repairs. Include in table <strong>for</strong>m <strong>and</strong>/or drawing <strong>for</strong>mat the following:<br />

• The occupancy classification <strong>of</strong> the building or portions <strong>of</strong> the building, related to new<br />

work as well as existing buildings, in accordance with Cali<strong>for</strong>nia Building St<strong>and</strong>ards<br />

Code (CBC) Chapter 3, <strong>and</strong> any specific use or occupancies in accordance with CBC<br />

Chapters 4 <strong>and</strong> 12;<br />

• The date <strong>of</strong> construction <strong>of</strong> the original building(<strong>for</strong> addition <strong>and</strong> renovation projects);<br />

• Whether the building is fully or partially sprinklered or unsprinklered. If partially<br />

sprinklered, show boundaries or parts that are sprinklered;<br />

• The type <strong>of</strong> fire alarm system within the building (i.e., full or partial smoke detection,<br />

etc.);<br />

• The area <strong>of</strong> the building <strong>and</strong> number <strong>of</strong> stories <strong>and</strong>/or new construction, by occupancy <strong>and</strong><br />

construction types, with a description <strong>of</strong> allowable increases, in accordance with CBSC<br />

Chapter 5;<br />

• The construction type classifications in accordance with CBC Chapter 6 <strong>and</strong> Table 601;<br />

• The location <strong>of</strong> the building by Seismic <strong>Design</strong> Category in accordance with CBC Chapter<br />

16 (or reference to structural in<strong>for</strong>mation); <strong>and</strong><br />

• Reference to OSHPD-accepted requests <strong>for</strong> Alternate Methods <strong>of</strong> Compliance, Alternate<br />

Means <strong>of</strong> Protection, or Program Flexibility, in accordance with CBC Section 104.11 <strong>and</strong><br />

Section 1224.2 Exception 2.<br />

Applicable Codes <strong>and</strong> Regulations<br />

List all codes <strong>and</strong> regulations that the project is required to meet in its construction, including the<br />

specific state <strong>and</strong> local regulations. The OSHPD Fire Life Safety Officer has particular interest in<br />

specific National Fire Protection Association (NFPA) Codes <strong>and</strong> St<strong>and</strong>ards, including applicable<br />

editions, pertinent to the project. Refer to CAN 1 <strong>for</strong> the current listing <strong>of</strong> en<strong>for</strong>ceable codes <strong>and</strong><br />

CBC Chapter 47 <strong>for</strong> the current applicable editions <strong>of</strong> the NFPA Codes <strong>and</strong> St<strong>and</strong>ards.<br />
