• OSHPD shall schedule a final state agency inspection <strong>of</strong> the work subsequent to the<br />

receipt <strong>of</strong> the responsible architect or engineer’s statement that the contract is per<strong>for</strong>med<br />

or substantially per<strong>for</strong>med.<br />

• The final approval <strong>of</strong> the construction shall be issued by OSHPD when:<br />

o All work has been completed in accordance with the approved plans <strong>and</strong><br />

specifications,<br />

o The required verified compliance reports <strong>and</strong> test <strong>and</strong> inspection reports have<br />

been filed with OSHPD, <strong>and</strong><br />

o All remaining fees have been paid to OSHPD.<br />

• Final approval shall be confirmed by a letter sent to the Department <strong>of</strong> Health Services<br />

with a copy to the applicant. The letter shall state that the work has been constructed in<br />

accordance with CBSC, Title 24, Cali<strong>for</strong>nia Code <strong>of</strong> Regulations.<br />

• Upon completion <strong>of</strong> the project, all copies <strong>of</strong> construction procedure records as required<br />

by Section 7-145(a)6 shall be transmitted to OSHPD. These shall include final verified<br />

reports by the AOR, various EORs (e.g., Electrical, Mechanical, <strong>and</strong> Structural), the IOR,<br />

<strong>and</strong> the contractor as well as written notice from the hospital asking that the project be<br />

closed <strong>and</strong> certification <strong>of</strong> the final construction cost <strong>and</strong> cost <strong>of</strong> radiology equipment<br />

installed. <strong>Project</strong>s are classified as Closed Without Compliance (CLWC) typically <strong>for</strong> two<br />

reasons:<br />

o Work was ab<strong>and</strong>oned after it began.<br />

o The closing paperwork was not properly filed. <strong>Project</strong>s that are logged into the<br />

OSHPD database as CLWC do not disappear with the passage <strong>of</strong> time. They<br />

require resolution <strong>of</strong> the conditions that resulted in the assigned status. Future<br />

work may be severely impacted by the existence <strong>of</strong> CLWC projects when the new<br />

projects have to rely on conditions that were created under the CLWC project. The<br />

CLWC status can be cleared retroactively but <strong>of</strong>ten with great ef<strong>for</strong>t.<br />
