Translation and validation of the Danish version of the<br />
Patient Rated Wrist Evaluation questionnaire<br />

Background: The need for validated outcome measures is increasing.<br />
The Patient Rated Wrist Evaluation (PRWE) is a 15- item wrist specific<br />
questionnaire used to measure the effect of clinical treatment of wrist<br />
injuries.<br />
Purpose: The purpose was to translate into Danish, validate and examine<br />
the unidimensionality of the Patient Rated Wrist Evaluation in<br />
patients with wrist fractures.<br />
Methods: Translation was done by an expert panel followed by evaluation<br />
of a lay panel and a field test on ten patients. Afterwards 60 patients<br />
treated for wrist fractures with osteosynthesis or conservatively were<br />
included. The patients answered the Patient Rated Wrist Evaluation and<br />
the Nottingham Health Profile (NHP) at 1 week and 6 weeks after start<br />
of treatment, and again the Patient Rated Wrist Evaluation three days<br />
after last control. Rasch analysis was conducted using RUMM2030 software<br />
to assess unidimensionality and to create a fitted model in order to<br />
convert ordinal scores to interval scores.<br />
Findings: Both lay panel comments and field test revealed issues not<br />
dealt with by the expert panel, including difficulty estimating average<br />
pain which was altered to estimating maximum pain. Cronbach’s alpha:<br />
0.94. Intraclass correlation coefficient: 0.88. Difference of Means: 5.7<br />
(CI: 1.12-10.37, P=0.017). Convergent validity at first and last control<br />
was for pain, 0.51 and 0.46, and physical mobility, 0.56 and 0.64, respectively.<br />
Rasch analysis showed unidimensionality. There was no Differential<br />
Item Functioning (DIF) for the time interval. There were indications<br />
of misfit of items, but these items displayed good content validity and<br />
were within acceptable parameters. A nomogram for conversion from<br />
ordinal score to interval score could be made.<br />
Conclusion: The translation resulted in a modified questionnaire that<br />
represents correct easy-understandable Danish. The questionnaire can be<br />
used in clinical and research settings regarding patients with wrist fractures,<br />
and both ordinal and interval scores should be reported.<br />
