“Report recommends pharmacists be<br />

granted the authority to prescribe!”<br />

Happy New Year! This news, which<br />

came in late November with the public<br />

release <strong>of</strong> the Health Pr<strong>of</strong>essions Regulatory<br />

Advisory Council (HPRAC)’s<br />

Report on Scope <strong>of</strong> Practice in the<br />

health pr<strong>of</strong>essions was thrilling to many<br />

and concerning to others.<br />

As you know, Pharmacy is one <strong>of</strong><br />

several health pr<strong>of</strong>essions engaged<br />

over the past year in consultations<br />

with the Health Pr<strong>of</strong>essions Regulatory<br />

Advisory Council (HPRAC) respecting<br />

its “Scope <strong>of</strong> Practice” review<br />

and most recently its review on<br />

“Non-Physician Prescribing and Use<br />

<strong>of</strong> drugs”. Over the summer many <strong>of</strong><br />

you participated in the <strong>College</strong>’s series<br />

<strong>of</strong> webcasts to better understand<br />

the details <strong>of</strong> the <strong>College</strong>’s submission<br />

on scope <strong>of</strong> practice and the tremendous<br />

feedback and support that pharmacists<br />

provided back to HPRAC was<br />

We would encourage you to look at<br />

the report. Comments are invited back<br />

to the Ministry by <strong>January</strong> 30, <strong>2009</strong>.<br />

...we believe all pharmacists have the knowledge, skills<br />

and ability to appropriately exercise their pr<strong>of</strong>essional<br />

judgement respecting any prescriptions they receive.<br />

The <strong>College</strong> is extremely pleased<br />

that HPRAC supported the proposed<br />

wording amendments to Pharmacy’s<br />

scope <strong>of</strong> practice statement to recognize<br />

and include the important role<br />

pharmacists play in medication therapy<br />

management. HPRAC has also<br />

recommended pharmacists be able<br />

to administer substances by both injection<br />

and inhalation to facilitate the<br />

provision <strong>of</strong> education and information<br />

to patients and their agents, and that<br />

pharmacists be permitted to pierce the<br />

tissue below the dermis ( finger pricking)<br />

when using glucose monitoring devices<br />

to demonstrate proper use and/or<br />

to monitor blood glucose levels in collaborative<br />

practices.<br />

Most significant is HPRAC’s recommendation<br />

that pharmacists be granted<br />

prescribing authority. The <strong>College</strong><br />

is very pleased with the recommendations<br />

which clearly demonstrate a<br />

recognition <strong>of</strong> the high level <strong>of</strong> training<br />

and unique expertise that pharmacists<br />

possess. There are two key parts to<br />

HPRAC’s recommendation respecting<br />

prescribing for pharmacists and it’s<br />

important that we understand the distinction.<br />

The first part <strong>of</strong> “prescribing”<br />

would involve modifying, adapting and/<br />

or extending an already written prescription<br />

that is based on an alreadymade<br />

and communicated diagnosis by<br />

another practitioner. These are the activities<br />

that the <strong>College</strong> initially proposed<br />

be included within the current<br />

controlled act <strong>of</strong> “dispensing “(without<br />

further authorization) because they do<br />

not involve initiation <strong>of</strong> a new prescription<br />

by a pharmacist. The second part<br />

<strong>of</strong> “prescribing” HPRAC recommends<br />

would involve pharmacists initiating<br />

new prescriptions for certain Schedule<br />

l drugs for minor ailments.<br />

There is a significant difference<br />

between a pharmacist “modifying,<br />

adapting or extending an already written/existing<br />

prescription” and a pharmacist<br />

“initiating a prescription for a<br />

Schedule l drug “, whether for a minor<br />

ailment or not. We resist making the<br />

distinction at our peril. Differentiating<br />

between the two will be especially important<br />

going forward, and identifying<br />

who can do what, and within what<br />

terms and conditions.<br />

To maintain some consistency<br />

amongst the various health pr<strong>of</strong>essions<br />

who prescribe now and in the future,<br />

HPRAC is proposing the creation <strong>of</strong><br />

specific drug lists, or lists <strong>of</strong> drug classes/categories<br />

that respective pr<strong>of</strong>essions<br />

would be able to prescribe. The<br />

<strong>College</strong> does not support the need for<br />

a defined list <strong>of</strong> drugs or drug classes or<br />

categories for those prescribing activities<br />

associated with medication therapy<br />

management-modifying, adapting,<br />

or extending already-written prescriptions.<br />

With the exception <strong>of</strong> narcotic<br />

and controlled substances where we<br />

could support an exception, we believe<br />

all pharmacists have the knowledge,<br />

skills and ability to appropriately exercise<br />

their pr<strong>of</strong>essional judgement respecting<br />

any prescriptions they receive.<br />

Confining pharmacists to a specific or<br />

limited list <strong>of</strong> drugs when exercising<br />

pr<strong>of</strong>essional judgement respecting prescriptions<br />

already written by another<br />

practitioner would be counterproductive<br />

, and in our view, would seriously<br />

limit the pharmacists ability and effectiveness<br />

in managing medication therapy<br />

for their patients<br />

The <strong>College</strong> could consider a defined<br />

list more appropriate for pharmacists<br />

or any emerging prescribing<br />

pr<strong>of</strong>essionals who initiate drug therapy.<br />

The <strong>College</strong> is concerned that<br />

all pharmacists do not currently have<br />

the necessary training, competency<br />

or confidence to conduct the level <strong>of</strong><br />

assessment needed before initiating<br />

a prescription drug for any ailment.<br />

Our public protection mandate leads<br />

us to proceed more cautiously in supporting<br />

pharmacist initiation <strong>of</strong> Schedule<br />

l drugs- first ensuring that additional<br />

education and training was available<br />

and mandatory for all pharmacists<br />

and then working with educators,<br />

pharmacists and other pr<strong>of</strong>essions to<br />

determine which ailments and drugs<br />

could appropriately and safely be routinely<br />

initiated by pharmacists.<br />

These are very exciting times! While<br />

ongoing and <strong>of</strong>ten competing economic<br />

challenges facing the retail businesses<br />

where Pharmacy is commonly practiced<br />

may continue, there are before<br />

pharmacists now unprecedented and<br />

tremendous opportunities that we<br />

must, as a health care pr<strong>of</strong>ession embrace<br />

with enthusiasm but also with<br />

the respectful caution that is needed<br />

to optimize the care <strong>of</strong> your patients in<br />

a safe, responsible way- the future <strong>of</strong><br />

our pr<strong>of</strong>ession depends on this. Be assured<br />

that the <strong>College</strong> will continue to<br />

do its part to ensure all pharmacists are<br />

able to work to their fullest ability and<br />

scope- the rest is up to you.<br />

pharmacist is a pharmacist<br />

is a pharmacist. That is<br />

what our provincial governments<br />

are telling us through the upcoming<br />

Agreement on Internal Trade<br />

revisions. Next year, a licensed pharmacist,<br />

or any licensed pr<strong>of</strong>essional for<br />

that matter, will be able to move from<br />

province to province according to the<br />

AIT. This means there will be no retraining<br />

or re-examination <strong>of</strong> pr<strong>of</strong>essionals<br />

when moving from province to<br />

province, except where necessary for<br />

public protection.<br />

On one hand the <strong>College</strong> will be expected<br />

to readily license pharmacists<br />

who are already licensed in another<br />

province. On the other hand, the <strong>College</strong><br />

is also responsible for making sure<br />

that every pharmacist who practices in<br />

<strong>Ontario</strong> is currently competent to practice<br />

and meet the standards <strong>of</strong> practice.<br />

No matter where a pharmacist practices<br />

it is the responsibility <strong>of</strong> each <strong>of</strong> us<br />

Next year, a licensed pharmacist, or any licensed<br />

pr<strong>of</strong>essional for that matter, will be able to move<br />

from province to province according to the AIT...<br />

as pr<strong>of</strong>essionals to have plan in place for<br />

continuing education to keep us current<br />

in our areas <strong>of</strong> practice.<br />

That is not as simple as it used to<br />

be. In the past, many pharmacists have<br />

been comforted by the fact that they<br />

did 15 or 20 continuing education units<br />

(CEUs) per year. But think <strong>of</strong> it – medical<br />

and drug information is renewing itself<br />

every 5 to 10 years! And the pr<strong>of</strong>ession<br />

is expanding its scope so that pharmacists<br />

will now be using their medication<br />

knowledge is more accountable<br />

way – through adapting, modifying and<br />

extending prescriptions. How can we<br />

keep up with 20 CEUs per year?<br />

The <strong>College</strong> has a requirement for<br />

every pharmacist to keep a record <strong>of</strong><br />

their continuous learning. The On-line<br />

CPD portal gives each pharmacist an<br />

opportunity to self-assess their knowledge<br />

and skills, and to build a learning<br />

portfolio which will keep them current<br />

with issues in their daily practice. As<br />

well, the OCP website contains several<br />

options for continuing education<br />

which pharmacists can access on a<br />

regular basis. The CPD portal, recently<br />

launched, is now available to all pharmacists<br />

to use as a tool for continuous<br />

pr<strong>of</strong>essional development. We also encourage<br />

you to plan your learning for<br />

the year in anticipation <strong>of</strong> an expanded<br />

scope. Check the OCP website for opportunities<br />

for continuing pr<strong>of</strong>essional<br />

development in your area and on-line.<br />

With the AIT agreement, and the<br />

review <strong>of</strong> scope <strong>of</strong> practice in <strong>Ontario</strong>,<br />

every pharmacists in <strong>Ontario</strong> will be<br />

accountable for demonstrating their<br />

knowledge and skills at a new level.<br />

Make <strong>2009</strong> the year for increased<br />

pr<strong>of</strong>essional development and pr<strong>of</strong>essional<br />

renewal.<br />

Council Approves Proposed<br />

Amendments To The<br />

General Operating By-Law<br />

The amendments to the Regulated<br />

Health Pr<strong>of</strong>essions Act, which will<br />

come into force on June <strong>2009</strong> as part<br />

<strong>of</strong> the Health System Improvements Act<br />

(HSIA), include changes to the section<br />

<strong>of</strong> the Procedural Code that pertain<br />

to the Register. Accordingly, Council<br />

discussed and approved amendments<br />

to Article I (Definitions), Article XII<br />

(Register) and Article XIII (Filing <strong>of</strong> Information<br />

by <strong>Pharmacists</strong>, Pharmacies<br />

and Health Pr<strong>of</strong>ession Corporations)<br />

<strong>of</strong> the by-law. These amendments expand<br />

the list <strong>of</strong> information that the<br />

public must have access to and direct<br />

that all the information on the Register<br />

(as stipulated in the Code as well<br />

as other information to be maintained<br />

in the Register in accordance with our<br />

by-laws) shall be posted on the <strong>College</strong>’s<br />

website.<br />

Furthermore, there was agreement<br />

that the <strong>College</strong> will interpret<br />

the Health System Improvements Act<br />

(HSIA) Register amendments to require<br />

the <strong>College</strong> to continue to have<br />

available to the public the results <strong>of</strong> the<br />

discipline and incapacity proceedings<br />

that are on the public Register as <strong>of</strong><br />

June 3, <strong>2009</strong> but not to include in the<br />

Register as <strong>of</strong> June 4, <strong>2009</strong>, results <strong>of</strong><br />

discipline and incapacity proceedings<br />

which have been removed as <strong>of</strong> June<br />

3, <strong>2009</strong>. This interpretation <strong>of</strong> the<br />

HSIA amendments is consistent with<br />

the position <strong>of</strong> other Health <strong>College</strong>s.<br />

Capital Budget Approved<br />

As reported in the previous issue <strong>of</strong><br />

Pharmacy Connection, at its meeting<br />

in September, Council agreed to defer<br />

approval <strong>of</strong> a capital budget for building<br />

and leaseholds pending consultation<br />

to determine how the current<br />

facilities owned and operated by the<br />

<strong>College</strong> could be reconfigured to increase<br />

the utility and efficiency.<br />

This spring, on the recommendation<br />

<strong>of</strong> the Building Committee, the<br />

<strong>College</strong> contracted the services <strong>of</strong><br />

Mayhew Workspaceworks, a real-estate<br />

and design firm to undertake an<br />

objective analysis <strong>of</strong> the space needs<br />

and expectations. Upon confirming<br />

the importance <strong>of</strong> the existing facilities<br />

to the organization, they explored<br />

the concept <strong>of</strong> utilizing the<br />

current space in a different way and<br />

have recommended an approach that<br />

would incorporate alternate work arrangements,<br />

including telecommuting<br />

into the <strong>College</strong>’s operations.<br />

Following a comprehensive interactive<br />

process, involving all levels <strong>of</strong><br />

management and staff, Mahew made<br />

recommendations respecting conversion<br />

<strong>of</strong> the space and the business<br />

processes. Levels <strong>of</strong> telecommuting<br />

were established for all existing job<br />

functions at the <strong>College</strong> with criteria<br />

attached to each. A two year transition<br />

period is proposed to evolve both<br />

the physical facility at 483 Huron<br />

Street as well as the technological<br />

supports for operations to enable<br />

the majority <strong>of</strong> the operations to be<br />

undertaken at home <strong>of</strong>fice locations.<br />

Based on these recommendations,<br />

Council approved an expenditure <strong>of</strong><br />

$1,800,000 for transitioning internal<br />

<strong>College</strong> operations to a telecommuting<br />

platform ($1.3 million – facility related;<br />

$0.5 million for work process<br />

conversion).<br />

The transition is aimed at maintaining<br />

or improving services provided<br />

by the <strong>College</strong> as the need to accelerate<br />

on-line services for staff translates<br />

to increased on-line services for the<br />

public, members and other stakeholders.<br />

Although many other business<br />

sectors have moved to telecommuting<br />

as a means <strong>of</strong> providing flexibility<br />

for staffing while reducing overhead<br />

costs for facility related expenses,<br />

our <strong>College</strong> was commended for its<br />

leadership among regulatory agencies<br />

in pursuing this option.<br />

For the next two years, it is anticipated<br />

that the space at 186 St.<br />

George would be utilized to temporarily<br />

hold staff dislocated from their<br />

workspace at 483 during construction.<br />

As we progress through the<br />

transformation <strong>of</strong> the 483 space, the<br />

Finance Committee will consider the<br />

appropriate action with respect to<br />

property on St. George Street.<br />

Submissions Update<br />

In early October, this <strong>College</strong>,<br />

together with other regulated health<br />

pharmacyconnection • <strong>January</strong>/<strong>February</strong> <strong>2009</strong><br />

colleges and pr<strong>of</strong>essional associations<br />

whose members may prescribe<br />

or administer drugs as part <strong>of</strong> their<br />

practice, were invited by HPRAC to<br />

a briefing respecting their review <strong>of</strong><br />

Non-Physician Prescribing and Administration<br />

<strong>of</strong> Drugs under the Regulated<br />

Health Pr<strong>of</strong>essions Act and<br />

were invited to submit comments respecting<br />

this matter by November 12,<br />

2008.<br />

While much <strong>of</strong> the information<br />

previously submitted in the “Scope<br />

<strong>of</strong> Practice for Pharmacy” documents<br />

applies to this review as well in this<br />

new review, the <strong>College</strong> focuses on<br />

the actual activities required for pharmacists<br />

to fully realize their roles in<br />

Medication Therapy Management,<br />

namely adapting, modifying and extending<br />

prescriptions, and administering<br />

drugs by inhalation and injection.<br />

In mid November, the Hon. David<br />

Caplan, Minister <strong>of</strong> Health and<br />

Long-Term Care <strong>of</strong> <strong>Ontario</strong> released<br />

Health Pr<strong>of</strong>essions Regulatory Advisory<br />

Council’s (HPRAC) Interim<br />

Report to the Minister <strong>of</strong> Health and<br />

Long-Term Care on Mechanisms to<br />

Facilitate and Support Interpr<strong>of</strong>essional<br />

Collaboration among Health<br />

<strong>College</strong>s and Regulated Health Pr<strong>of</strong>essionals:<br />

Phase ll, Part l. In this report,<br />

HPRAC has included its report<br />

and recommendations respecting<br />

the scope <strong>of</strong> practice review <strong>of</strong> four<br />

regulated health pr<strong>of</strong>essions, namely,<br />

Pharmacy, Physiotherapy, Midwifery,<br />

and Dietetics.<br />

The key recommendations included<br />

in the report are as follows:<br />

Support <strong>of</strong> a Scope <strong>of</strong> Practice Statement<br />

(HPRAC has supported the<br />

<strong>College</strong>’s proposals respecting adding<br />

the pharmacist’s role in Medication<br />

Therapy Management. HPRAC<br />

has also changed “provision <strong>of</strong> information<br />

relating to…..” to “providing<br />

education relating to their use”).<br />

Support <strong>of</strong> the addition <strong>of</strong> four new<br />

Controlled Acts (HPRAC has supported<br />

the addition <strong>of</strong> four new controlled<br />

or authorized acts, subject to<br />

terms and conditions proposed by<br />

the <strong>College</strong>. These include: prescribing,<br />

skin pricking, administration <strong>of</strong> a<br />

substance by injection and administration<br />

<strong>of</strong> a substance by inhalation).<br />

HPRAC further recommends that<br />

pharmacists have the authority to initiate<br />

prescriptions for minor ailments<br />

and for smoking cessation. Furthermore,<br />

pharmacists will now be authorized<br />

to order laboratory tests for<br />

Medication Therapy Management<br />

and monitoring purposes.<br />

HPRAC did not support pharmacists<br />

being authorized to perform<br />

routine injections or immunizations;<br />

or being authorized to initiate therapy<br />

for travel prophylaxis to prescribe<br />

Schedule ll and lll drugs solely for purposes<br />

<strong>of</strong> reimbursement under an insurance<br />

plan.<br />

Council was extremely pleased<br />

with the recommendations put<br />

forth by HPRAC and appreciates<br />

HPRAC’s recognition respecting the<br />

high level <strong>of</strong> training and education<br />

pharmacists in <strong>Ontario</strong> possess.<br />

The Ministry <strong>of</strong> Health and Long-<br />

Term Care has invited comments by<br />

the end <strong>of</strong> <strong>January</strong>, <strong>2009</strong> and, to this<br />

end, Council has directed that with<br />

respect to the recommendation respecting<br />

the establishment <strong>of</strong> lists <strong>of</strong><br />

specific drugs-categories or classes,<br />

the <strong>College</strong> indicate our support <strong>of</strong> a<br />

list <strong>of</strong> defined drug classes or categories<br />

ONLY where pharmacists initiate<br />

prescriptions for Schedule l drugs.<br />

Council also expressed strong support<br />

<strong>of</strong> the inclusion <strong>of</strong> objects <strong>of</strong> the<br />

<strong>College</strong> to ensure collaboration between<br />

pr<strong>of</strong>essions but does not support<br />

the mandatory inclusion <strong>of</strong> other<br />

pr<strong>of</strong>essions on our Pharmacy Standards<br />

Committee. The <strong>College</strong> will<br />

however, indicate our commitment to<br />

obtaining and utilizing the input and<br />

expertise <strong>of</strong> those pr<strong>of</strong>essions who<br />

currently prescribe and perform lab<br />

tests as we go forward in developing<br />

and establishing practice standards to<br />

support pharmacists in the new authorized<br />

acts and scope <strong>of</strong> practice.<br />

CPSO Dispensing Policy<br />

Endorsed<br />

The <strong>College</strong> <strong>of</strong> Physicians and Surgeons<br />

<strong>of</strong> <strong>Ontario</strong> (CPSO), in collaboration<br />

with this <strong>College</strong>, has developed<br />

a policy on dispensing for<br />

those physicians who dispense drugs,<br />

or who are considering adding dispensing<br />

to their practice. Council endorsed<br />

the proposed policy, noting<br />

that both the Pr<strong>of</strong>essional Practice<br />

and the Executive Committees were<br />

<strong>of</strong> the opinion that physicians be held<br />

to the same standard as pharmacists<br />

with respect to the controlled act <strong>of</strong><br />

dispensing in order to ensure public<br />

safety.<br />

Advertising Regulations<br />

Upheld<br />

Following the release <strong>of</strong> a report by<br />

the Competition Bureau as it related<br />

to the Pharmacy pr<strong>of</strong>ession, the<br />

<strong>College</strong> undertook a review <strong>of</strong> the<br />

current advertising regulation to determine<br />

whether any revisions or<br />

amendments are appropriate.<br />

The <strong>College</strong>’s current advertising<br />

egulation, made under both the<br />

Pharmacy Act and the Drug and Pharmacies<br />

Regulation Act (DPRA), were<br />

reviewed at length by the <strong>College</strong>’s<br />

Pr<strong>of</strong>essional Practice Committee and<br />

also debated extensively at Council,<br />

with the conclusion that the current<br />

provisions <strong>of</strong> the advertising regulation<br />

are appropriate and should be<br />

retained.<br />

In particular, the public members<br />

<strong>of</strong> Council expressed serious concerns<br />

respecting the advertising <strong>of</strong> individual<br />

prescription products as currently<br />

occurs in the USA and strongly<br />

believe it is not in the public interest<br />

to permit pharmacists to advertise<br />

sole prescription products to the public<br />

as loss leaders. While the current<br />

requirement to include at least 15 different<br />

drugs, 10 <strong>of</strong> which each belong<br />

to a different classification may appear<br />

onerous, there was strong consensus<br />

that this requirement actually<br />

protects the public from a pharmacy<br />

pushing or promoting one Rx product<br />

over another. Further, it was suggested<br />

that this may actually encourage<br />

competition while supporting<br />

the public in making their choices respecting<br />

the provision <strong>of</strong> pharmacy<br />

services in accordance with the level<br />

<strong>of</strong> pharmaceutical service and care<br />

provided rather than on price <strong>of</strong> product<br />

alone.<br />

Library Requirements For<br />

Pharmacies<br />

Council endorsed a recommendation<br />

made by the Accreditation Committee<br />

to add a new reference from MediResource,<br />

Patient Connect DDI<br />

to the patient counselling category<br />

and is in agreement with the Accreditation<br />

Committee that it would<br />

be beneficial to members in their care<br />

<strong>of</strong> patients. Current library requirements<br />

can be found on the <strong>College</strong>’s<br />

website at www.ocpinfo.com<br />

Ait/Labour Mobility<br />

Council received for information a<br />

comprehensive presentation by <strong>of</strong>ficials<br />

from the Ministry <strong>of</strong> Training<br />

<strong>College</strong>s and University respecting<br />

AIT and <strong>Ontario</strong>-Quebec labour<br />

mobility agreements. Included in the<br />

presentation was a review <strong>of</strong> the process<br />

for implementing the two agreements,<br />

including key milestones and<br />

timing. An overview <strong>of</strong> the key elements<br />

<strong>of</strong> the AIT and implications for<br />

regulatory bodies was also provided.<br />

Following the presentation and after<br />

extensive discussion, Council recommended<br />

that staff prepare and provide<br />

to the Ministry, a business case<br />

that demonstrates why this <strong>College</strong>’s<br />

requirements for a jurisprudence<br />

exam and an International Pharmacy<br />

Graduate program should be considered<br />

necessary and valid requirements<br />

for labour mobility.<br />

To this end, it was noted that NA-<br />

PRA, the National Association <strong>of</strong><br />

Pharmacy Regulatory Authorities,<br />

is working concomitantly with the<br />

regulatory authorities on the Mutual<br />

Recognition Agreement (MRA)<br />

which will also serve to ensure labour<br />

mobility <strong>of</strong> pharmacists in Canada.<br />

The Agreement, which establishes<br />

the conditions under which a pharmacist<br />

who is licensed/registered in<br />

one Canadian jurisdiction will have his<br />

/her qualifications recognized in another<br />

Canadian jurisdiction, will support<br />

and enhance the federal agreements<br />

for the pr<strong>of</strong>ession.<br />

Remote Dispensing/<br />

E-Prescribing<br />

Council considered a legal opinion<br />

as to whether the current provisions<br />

<strong>of</strong> the DPRA (Drug and Pharmacies<br />

Regulation Act) permit remote<br />

dispensing and e-prescribing and<br />

agreed with the Executive Committee’s<br />

recommendation that this matter<br />

be referred, including the issue <strong>of</strong><br />

any amendments that would need<br />

to be made to the DPRA, to the Accreditation<br />

Committee for further<br />

consideration, and further that recommendations<br />

respecting any legislative/regulatory<br />

amendments to be<br />

brought back to Council for further<br />

discussion.<br />

Strategic Plan Update<br />

Progress continues towards meeting<br />

the goals and objectives set out in the<br />

Strategic Plan and Council received<br />

the progress report <strong>of</strong> action taken by<br />

all <strong>College</strong> areas since the September<br />

Council Meeting. It was noted that<br />

activities set in March 2006 are expected<br />

to reach completion in <strong>2009</strong><br />

when Council will embark upon a<br />

new Strategic Plan. The <strong>College</strong> has<br />

engaged the services <strong>of</strong> Ms. Elinor<br />

Caplan to facilitate this process and<br />

a report from this Strategic Planning<br />

Retreat will be presented to Council<br />

in June <strong>2009</strong> for ratification.<br />

No form will be mailed to you, however email reminders<br />

will be sent. Please ensure the <strong>College</strong> has your current<br />

email address. Please refer to the attached brochure for<br />

more information on how to complete your online renewal.<br />

regardless <strong>of</strong> payment method, through the information<br />

requiring verification and/or updating. We have been<br />

in contact with the pharmacy chains and worked out a<br />

process whereby the Confirmation <strong>of</strong> Information Renewal<br />

submitted to them by a pharmacist will suffice as notification<br />

that a pharmacist has completed the information renewal<br />

portion <strong>of</strong> their annual renewal. Employers will, in<br />

turn, provide a list to the <strong>College</strong> <strong>of</strong> all <strong>of</strong> these pharmacists<br />

when submitting payment.<br />

Information Renewal – Step 1<br />

• To begin your online renewal, go to www.ocpinfo.com,<br />

click on the Member Login button(as seen above).<br />

• Enter your User ID (your OCP number) and your<br />

password.<br />

The first time you login and until you change it, your<br />

password is the last six digits <strong>of</strong> your social insurance number.<br />

After your initial log in, we encourage you to change<br />

your password. Once you have successfully logged in,<br />

click on Pharmacist Renewal on the left hand side <strong>of</strong> the<br />

screen. Once you complete and submit this first step <strong>of</strong><br />

your renewal, print/save a copy <strong>of</strong> your Confirmation <strong>of</strong><br />

Information Renewal.<br />

After a review <strong>of</strong> our renewal processes, we realized<br />

that many people were not taking advantage <strong>of</strong> completing<br />

their renewal online because they preferred to pay by<br />

cheque or their pharmacy was paying their renewal on<br />

Payment – Step 2<br />

Payment by Credit Card or INTERAC can be made<br />

online in a secure environment digitally protected by<br />

Moneris .<br />

Your Confirmation <strong>of</strong> Information Renewal must be printed<br />

Late Payments<br />

Late payments are subject to a late fee <strong>of</strong> $105 ($100 + $5<br />

GST) (if paid within 30 days after the due date) or $157.50<br />

(if paid more than 30 days after the due date). This includes<br />

cheques that are received early but postdated after<br />

March 10, 2008. Late payments are not processed until<br />

the late payment fee has been received.<br />

Move from Part A to Part B <strong>of</strong><br />

the <strong>College</strong>’s Register<br />

To elect to Part B <strong>of</strong> the <strong>College</strong> register please send an<br />

email to ocpclientservices@ocpinfo.com indicating your<br />

desire to move to Part B <strong>of</strong> the register. <strong>Pharmacists</strong> in<br />

Part B <strong>of</strong> the register are not required to obtain personal<br />

pr<strong>of</strong>essional liability insurance or participate in practice review.<br />

The renewal fee for a Part B pharmacist is $282.40.<br />

Member Emeritus<br />

Any pharmacist who has practiced continually in good<br />

standing in <strong>Ontario</strong> and/or other jurisdictions for at least<br />

25 years can voluntarily resign from the Register and<br />

make an application for the Member Emeritus designation.<br />

Members Emeritus are not permitted to practice<br />

pharmacy in <strong>Ontario</strong> but will be added to the roll <strong>of</strong> persons<br />

so designated, receive a certificate and continue to<br />

receive Pharmacy Connection at no charge.<br />

Once you have completed your information filing<br />

online and your payment has been processed,<br />

your wallet card and income tax receipt will be<br />

mailed to your residence.<br />

Personal Pr<strong>of</strong>essional<br />

Liability Insurance<br />

Mandatory for Part A pharmacists<br />

It is a mandatory requirement <strong>of</strong> your annual renewal to<br />

update the details <strong>of</strong> your personal pr<strong>of</strong>essional liability<br />

insurance. You will be able to update your insurance<br />

information while completing your online renewal. The<br />

list <strong>of</strong> insurance products and broker organizations that<br />

satisfy the criteria prescribed in <strong>College</strong> by-law is listed<br />

below. Members should be cautious if considering any<br />

other insurance products. If your insurance product is not<br />

on the pre-approved list you will be required to provide<br />

the <strong>College</strong> with additional details <strong>of</strong> your policy. It is the<br />

member’s responsibility to ensure that they have<br />

compliant insurance and that their insurance is fully<br />

portable regardless <strong>of</strong> their employment status.<br />

QI understand that when I register with the <strong>College</strong><br />

there is an annual fee that I will have to pay. Can<br />

you tell me what the fee is, what it is for, and what<br />

benefit I will receive in return for this fee?<br />

Yes, there is an annual registration fee that all members <strong>of</strong><br />

the <strong>College</strong> must pay. The current proposal is that Pharmacy<br />

Technicians should pay a fee that is two thirds <strong>of</strong> what<br />

pharmacists pay. Based on the current fee structure, the<br />

amount would be $358.57.<br />

This fee is not set in relation to the pr<strong>of</strong>ession’s earning<br />

potential, but is based on the costs associated with self-regulation.<br />

These costs include activities such as registering new<br />

members, maintaining a continuing competency program,<br />

ensuring there are standards <strong>of</strong> practice, and investigating<br />

and resolving any practice concerns that are brought forward.<br />

In fact, there is an argument to be made that the fee<br />

should be consistent for all members <strong>of</strong> the <strong>College</strong>. In setting<br />

the fee, however, the Council considered that the scope<br />

<strong>of</strong> practice for pharmacy technicians is not as broad as that<br />

<strong>of</strong> a pharmacist in Part A <strong>of</strong> the register.<br />

It is difficult to describe the benefit <strong>of</strong> paying these fees.<br />

To be a regulated pr<strong>of</strong>essional means one has an exclusive<br />

right and privilege to use the title <strong>of</strong> the pr<strong>of</strong>ession and to<br />

perform a function for society that others are not permitted<br />

to do. A registered pharmacy technician will have the authority<br />

and independent accountability to perform the technical<br />

functions related to dispensing and compounding <strong>of</strong><br />

drugs. As a member <strong>of</strong> a self-regulated pr<strong>of</strong>ession, you have<br />

the added privilege <strong>of</strong> being held to account by a body <strong>of</strong><br />

your peers, as opposed to others who may not be familiar<br />

with the realities <strong>of</strong> pr<strong>of</strong>essional practice.<br />

QWhen I become registered with the <strong>College</strong>, will<br />

I also have to purchase pr<strong>of</strong>essional liability<br />

insurance, as pharmacists do?<br />

It has not been proposed that pharmacy technicians be required<br />

to maintain personal pr<strong>of</strong>essional liability insurance<br />

(as is now required for pharmacists and interns). Up until<br />

last year, pharmacists were not required to have personal<br />

coverage, because they were covered under the policies at<br />

their place <strong>of</strong> employment. The new requirement for pharmacists<br />

and interns to have their own policy relates to the<br />

expanding roles and new environments that pharmacists are<br />

working in, and their need for coverage to be more mobile.<br />

At this time, it is expected that pharmacy technicians will be<br />

covered within their place <strong>of</strong> employment, just as pharmacists<br />

were in the past. The <strong>College</strong> will continue to monitor<br />

this expectation and make adjustments if needed.<br />

Although it is not required, the <strong>College</strong> strongly recommends<br />

personal pr<strong>of</strong>essional liability insurance for all members.<br />

At a minimum, pharmacy technicians should confirm<br />

that they are covered by their employer.<br />

QI passed the certification exam several years ago.<br />

What benefit has that actually given me since I<br />

am still required to complete the bridging education<br />

program and entry-to-practice and jurisprudence<br />

exams to become registered with the <strong>College</strong>?<br />

The certification exam was put into place to address a concern<br />

that pharmacists expressed regarding the diversity <strong>of</strong><br />

training programs for pharmacy technicians. With applicants<br />

for pharmacy technician positions having trained on<br />

the job, at a private career college, or at a community college,<br />

employers were unsure how to compare among them.<br />

Pharmacy technicians who achieved certification demonstrated<br />

they had at least the consistent level <strong>of</strong> knowledge<br />

required to pass the exam. Over time, many employers<br />

came to rely on the certification as a requirement to demonstrate<br />

an applicant was qualified for the job. Individuals<br />

who maintained their certification also demonstrated a<br />

commitment to continuing education.<br />

Those pharmacy technicians who have completed the<br />

certification exam have the advantage <strong>of</strong> satisfying the first<br />

requirement for registration, having already demonstrated<br />

that they also have the knowledge necessary to enter the<br />

bridging education program.<br />

The <strong>College</strong> also considers those who have achieved and<br />

maintained their certification over the years as leaders in<br />

the pr<strong>of</strong>ession. Through their commitment, these individuals<br />

helped demonstrate the pr<strong>of</strong>ession’s readiness for selfregulation.<br />

We believe this is another important benefit <strong>of</strong><br />

your certification.<br />

The <strong>College</strong> bid a fond farewell to Carol Culhane, Administrative<br />

Assistant in the Continuing Competence program.<br />

Carol had been with the <strong>College</strong> since 2005 and has decided<br />

to focus her efforts on her Shiatsu practice and teaching.<br />

Kathy Vesterfelt joined the <strong>College</strong> at the beginning <strong>of</strong> December<br />

as the Senior Policy Advisor for the <strong>College</strong>. Kathy<br />

is an <strong>Ontario</strong> pharmacy graduate with experience in hospital<br />

and community pharmacy, and for the past nine years<br />

has worked at Health Canada in a variety <strong>of</strong> roles including<br />

that <strong>of</strong> Policy Analyst and Project Manager.<br />

Most recently she was the Acting Manager <strong>of</strong> the Evaluation<br />

and Authorization Division <strong>of</strong> the Office <strong>of</strong> Controlled<br />

Substances where she worked on, among other things, the<br />

exemption for pharmacists providing methadone. Kathy has<br />

also been a Teaching Associate at U <strong>of</strong> T (Faculty <strong>of</strong> Pharmacy)<br />

and a consultant for Health Canada in the First Nations<br />

and Inuit Health Branch.<br />

Margaret Poon has recently joined the <strong>College</strong> as the Records<br />

and Information Analyst. Margaret has 20 years <strong>of</strong><br />

records management experience having worked with the<br />

City <strong>of</strong> Toronto for over thirteen years and then moved to<br />

another regulatory <strong>College</strong> where she spent over five years<br />

as their Manager, Information Management, responsible for<br />

records management, library, intranet and statistics. Margaret<br />

has her BA in Economics and Mathematics and her<br />

Masters <strong>of</strong> Library Sciences.<br />

Dean Saeidvafa joined the <strong>College</strong> at the beginning <strong>of</strong> December<br />

as the Help Desk Specialist. Dean has more than 12<br />

years <strong>of</strong> extensive experience in desktop support, operating<br />

systems and applications support. Dean has a B. Sc. in<br />

Applied Computational and Mathematical Science, his certification<br />

as a Micros<strong>of</strong>t Certified System Engineer and his<br />

diploma as Information Technology Pr<strong>of</strong>essional.<br />

I successfully completed my Pharmacy degree at<br />

Q the University <strong>of</strong> Toronto Unfortunately, I failed<br />

one part <strong>of</strong> the PEBC Qualifying Exam. I thought I<br />

needed the exam before starting internship. Can you<br />

clarify when I can start my internship training?<br />

One requirement for starting internship is the completion<br />

<strong>of</strong> an undergraduate pharmacy program accredited by the<br />

Canadian Council for the Accreditation <strong>of</strong> Pharmacy Programs.<br />

OCP must confirm that you have the following:<br />

1. successful completion <strong>of</strong> your U <strong>of</strong> T Pharmacy<br />

program<br />

2. a preceptor who meets the structured practical training<br />

(spt) preceptor criteria<br />

3. a training site that meets the spt site criteria<br />

4. appropriate documentation, and<br />

5. personal pr<strong>of</strong>essional liability insurance.<br />

Once we notify you that you have met these requirements,<br />

you are entitled to start your internship training.<br />

The PEBC Qualifying Exam, Parts I & II is not necessary<br />

for internship, but must be completed before you can be<br />

registered as a pharmacist. For more information on preceptor<br />

QI am an internationally trained pharmacist who<br />

went to a panel <strong>of</strong> the Registration Committee<br />

to request an exemption from studentship. It seems<br />

unfair to me that the <strong>College</strong> allows U <strong>of</strong> T students<br />

to start internship without the PEBC Qualifying Exam,<br />

Parts I & II but that international pharmacy graduates<br />

are required to pass the exam before going to a panel<br />

Could you explain why I am being treated differently?<br />

The key point here is not that you are an international pharmacy<br />

graduate, but that you are requesting an exemption<br />

from one <strong>of</strong> the entry-to-practice requirements, namely,<br />

studentship. For international pharmacy graduates, studentship<br />

is comprised <strong>of</strong> two elements: completion <strong>of</strong> 16<br />

weeks <strong>of</strong> the IPG (International Pharmacy Graduate) Program,<br />

plus 16 weeks <strong>of</strong> structured practical training at the<br />

student level.<br />

International pharmacy graduates, who have successfully<br />

completed the IPG program and have completed structured<br />

practical training are treated the same as graduates<br />

<strong>of</strong> the B.Sc. Phm program at the University <strong>of</strong> Toronto,<br />

and may start their internship without the PEBC Qualifying<br />

Exam. However, everyone is required to successfully<br />

complete both parts <strong>of</strong> the PEBC Qualifying Exam in order<br />

to be licensed.<br />

A panel <strong>of</strong> the Registration committee would require<br />

some supporting evidence to grant an exemption from the<br />

IPG program or a reduction in studentship. The successful<br />

completion <strong>of</strong> the PEBC Qualifying Exam, Parts I & II is a<br />

key piece <strong>of</strong> evidence panels have considered in the past.<br />

QThere are several drugs listed in the National<br />

Drug Schedules, Schedule I that do not have a<br />

“Pr” symbol written on the upper left corner <strong>of</strong> the<br />

label <strong>of</strong> the prescription bottle. Are these considered<br />

prescription drugs?<br />

Yes. <strong>Ontario</strong> adopted the National Drug Scheduling System<br />

in 1999, replacing provincial schedules with the National<br />

Drug Schedules, and all drugs listed in Schedule I require a<br />

prescription in order to sell.<br />

The “Pr” symbol found on many Schedule F drugs is<br />

unique to the Food and Drugs Act (FDA) and is required by<br />

section C.01.004 (b)(i) for drugs listed Part C and D <strong>of</strong> the<br />

Regulations to the FDA. Schedule F is included in Part C.<br />

All the drugs listed in Schedule F <strong>of</strong> the FDA are listed as<br />

Schedule I drugs in the National Drug Schedules. Any drug<br />

not listed in Schedule F <strong>of</strong> the FDA is reviewed by the National<br />

Drug Scheduling Advisory Committee to determine<br />

the level <strong>of</strong> intervention and advice necessary for the safe,<br />

effective use <strong>of</strong> that drug, and then placed in the appropriate<br />

schedule. The most common examples are muciprocin<br />

(Bactroban®) and quinine capsules.<br />

QWhen the National Drug Scheduling Advisory<br />

Committee approves a change in schedule,<br />

i.e., a Schedule I drug moves to Schedule II or III,<br />

or becomes unscheduled, is there a period <strong>of</strong> time<br />

before this change is effective in <strong>Ontario</strong>?<br />

No. <strong>Ontario</strong> uses the National Drug Schedules as its<br />

provincial drug schedule and they are embedded in the<br />

Drug and Pharmacies Regulation Act. This means that <strong>Ontario</strong>,<br />

in effect, schedules by reference. When the NDSAC<br />

approves a drug schedule or a drug schedule change, the<br />

change takes effect immediately in <strong>Ontario</strong>.<br />

QCan a pharmacist deliver a Schedule II narcotic<br />

preparation such as acetaminophen with<br />

codeine 1/8 gr?<br />

A pharmacist can deliver any Schedule II product, provided<br />

that he or she has met the requirements for sale <strong>of</strong> a Schedule<br />

II product. The pharmacist must be satisfied that the<br />

sale is appropriate prior to approving it for delivery.<br />

The Executive Officer issued a notice on Oct 2,<br />

2008 (BBS #8085) notifying pharmacists <strong>of</strong> the<br />

termination <strong>of</strong> the existing HNS agreement, effective<br />

Jan. 15, <strong>2009</strong>, and issuance <strong>of</strong> a new agreement.<br />

Failure to execute the new agreement will result<br />

in the suspension <strong>of</strong> that pharmacy operator’s<br />

account on Jan. 15, <strong>2009</strong>. Questions about the new<br />

When checking prescriptions, pharmacists must consider<br />

the appropriateness <strong>of</strong> the dosage being prescribed.<br />

This can only be done if the pharmacist<br />

is aware <strong>of</strong>, or considers the indication for use or purpose <strong>of</strong> the<br />

medication being prescribed. For example, when codeine is<br />

prescribed for the treatment <strong>of</strong> acute pain, the reccommended<br />

adult dosage is 15 to 60 mg orally every four to six hours. 1 However,<br />

when used as a cough suppressant, the recommended<br />

dosage is 5 to 20 mg orally every four to eight hours.2 Yet, the<br />

indication for use or purpose <strong>of</strong> the medication is not usually<br />

included on prescriptions. This missing piece <strong>of</strong> information is<br />

<strong>of</strong>ten a contributing factor to medication errors.<br />

Case:<br />

prescription, the pharmacist on this occasion observed that the<br />

dosage was not consistent with the usual dosage used in the<br />

treatment <strong>of</strong> herpes labialis or other common viral infections.<br />

The original prescription was therefore checked to confirm the<br />

prescriber’s intent. On checking the original prescription, the<br />

dosing error was identified. The correction was made, and<br />

the patient given eight Valtrex® 500 mg tablets with instructions<br />

to take four tablets (2000 mg) twice daily. The patient<br />

was very upset upon hearing <strong>of</strong> the error. She stated that she<br />

“thought something was wrong, and questioned the pharmacist.”<br />

Unfortunately, the pharmacist at that time read the prescription<br />

and reassured the patient that the physician did indeed<br />

prescribe two tablets twice daily.<br />

Possible Contributing Factors:<br />

• The pharmacy stocked the 500 mg strength <strong>of</strong> Valtrex® and<br />

not the 1000 mg.<br />

• The technician failed to adjust the directions for use to compensate<br />

for the change in strength.<br />

• The indication for use or purpose <strong>of</strong> the medication was not<br />

included on the prescription.<br />

• The initial pharmacist did not consider the possible indication<br />

for use to ensure appropiateness <strong>of</strong> the dose.<br />

• Despite concerns raised by the patient, the pharmacist<br />

failed to double check all components <strong>of</strong> the prescription for<br />

accuracy.<br />

The above prescription was written for a patient for the<br />

treatment <strong>of</strong> cold sores (herpes labialis). The prescription<br />

was taken to a local pharmacy for processing.<br />

On entering the prescription into the computer, the pharmacy<br />

technician entered Valtrex® 500 mg tablets. However,<br />

the directions for use was not adjusted and therefore entered<br />

as two tablets twice daily. When checking the prescription,<br />

the pharmacist did not detect the error. A total <strong>of</strong> four Valtrex®<br />

500 mg tablets were therefore dispensed instead <strong>of</strong> four<br />

1000 mg tablets or eight 500 mg tablets with the appropiate<br />

instructions for use.<br />

A few months later, the patient requested a refill <strong>of</strong> the Valtrex®<br />

tablets. A pharmacy technician processed the refill and<br />

gave it to the pharmacist for checking. On checking the refill<br />

Recommendations:<br />

• Whenever possible, dispense the same strength or concentration<br />

<strong>of</strong> the medication as prescribed. The directions for use<br />

would therefore be the same given to the patient by the prescriber.<br />

Clearly explain any changes to the patient.<br />

• Always consider the possible indication for use when checking<br />

prescriptions for appropiateness and accuracy.<br />

• Investigate thoroughly all concerns raised by the patient.<br />

• Appropiate steps should be taken by all stakeholders to ensure<br />

that the indication for use or purpose <strong>of</strong> the medication<br />

is included on all prescriptions. This is certainly a patient<br />

safety issue.<br />

The Hong Kong Department <strong>of</strong> Health warned consumers not to buy or use Lu Quan because it contains<br />

undeclared glibenclamide and sildenafil. The Hong Kong Department <strong>of</strong> Health warned consumers not to<br />

buy or use Fat Killer, Carbohydrate Cut and Sugar-Carbohydrate Cut because they contain sibutramine and<br />

an unauthorised substance similar in structure to sibutramine, and Zhuang Yao Gu Shen Capsule because it<br />

contains sildenafil.<br />

Glaxo Operations UK Limited and Shopper’s Drug Mart Specialty Health Network are voluntarily recalling 2 lots<br />

<strong>of</strong> Argatroban vials (lot numbers C347625 and C363385) from the Canadian marketplace.<br />

The U.S. FDA warned consumers not to buy or use Viapro because it contains an unauthorised substance<br />

similar in structure to sildenafil. Sildenafil is a prescription drug used in the treatment <strong>of</strong> erectile dysfunction and<br />

should only be used under the supervision <strong>of</strong> a health care practitioner. The U.S. Food and Drug Administration<br />

informed consumers <strong>of</strong> a voluntary manufacturer recall <strong>of</strong> these 12 products because they contain human<br />

placenta, aristolochic acid and/or ephedra, and may pose serious health risks. All 12 products are manufactured<br />

by Jen-On Herbal Science International Inc. (also known as Herbal Science International Inc.). Consumers who<br />

had purchased these products were advised to discontinue their use immediately and return them to the place<br />

<strong>of</strong> purchase for a full refund.<br />

Infection-related deaths were reported with the use <strong>of</strong> MabCampath (alemtuzumab) following initial<br />

combination therapy with anti-leukemia drugs which also suppress the body’s immune response. Serious<br />

infections, including infection-related deaths have been reported in association with the unauthorized use <strong>of</strong><br />

MabCampath as consolidation therapy following combination treatment with other immunosuppressive agents<br />

for B-cell chronic lymphocytic leukemia.<br />

Some glucose monitor systems may report falsely elevated readings associated with the use <strong>of</strong> medical products<br />

which contain or are metabolised to maltose, galactose and xylose. Health Canada provides recommendations<br />

to minimize the risks <strong>of</strong> these device-drug interactions.<br />

Hill-Rom issued an Urgent Medical Device Correction letter to notify customers <strong>of</strong> safety information regarding<br />

Liko Uno Patient Lift. The actuator <strong>of</strong> the lift may separate from the lift leading to the patient fall. Users need to<br />

take the affected units out <strong>of</strong> service immediately and contact the company for corrective options.<br />

Fatal adverse events have occurred in patients with moderate hepatic impairment and advanced solid tumours<br />

treated with Tarceva (erlotinib).<br />

Health Canada is advising consumers <strong>of</strong> the outcome <strong>of</strong> its review <strong>of</strong> cough and cold medicines for children<br />

under the age <strong>of</strong> 12. Manufacturers are required to re-label over the counter cough and cold medicines that<br />

have dosing information for children to indicate that these medicines should not be used in children under 6.<br />

Cases <strong>of</strong> eye inflammation have been reported following <strong>of</strong>f-label intravitreal injection <strong>of</strong> Avastin lot<br />

B3002B028. There has been no unusual reporting pattern for this lot with authorized use in 2008. It meets all<br />

specifications for lot release.<br />

Health Canada is informing diabetic patients taking Actos (pioglitazone hydrochloride) <strong>of</strong> recent changes to the<br />

prescribing information for this product. Health Canada has worked with the manufacturer to strengthen the<br />

labeling around heart failure and emphasize proper use. The current Product Monograph for Actos contains the<br />

new or updated information.<br />

Regional Continuing Education (CE)<br />

Co-ordinator Role Description<br />

Purpose<br />

The Regional CE Co-ordinators assist in the identification<br />

<strong>of</strong> CE needs <strong>of</strong> pharmacists in their region and help organize<br />

CE events.<br />

The CE Co-ordinator works with the <strong>Ontario</strong> <strong>College</strong><br />

<strong>of</strong> <strong>Pharmacists</strong> (OCP), the <strong>Ontario</strong> <strong>Pharmacists</strong>’ Association<br />

(OPA) and, where feasible, with the local pharmacists’<br />

association. Depending on the needs <strong>of</strong> the region,<br />

coordinators may work in teams <strong>of</strong> two or more.<br />

Goal: To provide live, evidence-based CE events for pharmacists<br />

throughout <strong>Ontario</strong><br />

Outcome: Each CE Co-ordinator will organize at least<br />

two events per year in their region<br />

Responsibilities<br />

With the support <strong>of</strong> OCP and OPA you will:<br />

1. Identify CE Needs <strong>of</strong> <strong>Pharmacists</strong> in Your Region<br />

Through your direct contact with pharmacists in your region,<br />

you will identify and prioritize their learning needs.<br />

CE programs which meet the needs <strong>of</strong> the maximum<br />

number <strong>of</strong> pharmacists will be selected. If you are new to<br />

your region, OCP will supply you with a list <strong>of</strong> pharmacists<br />

and contact information so you can contact pharmacists<br />

in your region.<br />

2. Identify Topics and Speakers for Presentation in<br />

Your Region<br />

Having identified CE topics <strong>of</strong> interest, you may seek<br />

out local specialists in your area to provide education for<br />

your members. Alternatively, you may liaise with Coordinators<br />

from other regions to provide leads for topics<br />

and speakers. OCP will also advise you <strong>of</strong> any potential<br />

CE programs to be delivered in your region.<br />

3. Organizing CE Presentations<br />

CE Co-ordinators develop the program’s budget, obtain<br />

financial sponsorship if needed, organize meetings<br />

(facilities, refreshments, AV needs, marketing the<br />

event), request program accreditation / CEU assignment,<br />

collect registration fees where applicable, serve<br />

as moderator, recruit members, etc, as required.<br />

OCP will help to publicise the program both on our<br />

website and in Pharmacy Connection.<br />

Time Commitment<br />

The amount <strong>of</strong> time involved in becoming a CE Co-ordinator<br />

varies. It will depend on the number <strong>of</strong> presentations<br />

you are able / willing to organize in your region. As a rough<br />

approximation, assume about 10 hours <strong>of</strong> involvement per<br />

program presented and one full-day meeting per year to<br />

network with other CE Co-ordinators.<br />

OCP is currently looking for regional CE coordinators in district areas 9, 17, 23, 25, and for an<br />

Associate coordinator in district 14<br />

2008. Over 1500 pharmacy technicians<br />

took part in the <strong>College</strong>’s<br />

final <strong>of</strong>fering <strong>of</strong> the exam.<br />

Anyone who has not completed the OCP certification<br />

exam but would still like to pursue registration with the <strong>College</strong><br />

in the future will need to complete the Pharmacy Examining<br />

Board <strong>of</strong> Canada’s Evaluating Exam. Details about the<br />

PEBC exam can be found at www.pebc.ca .<br />

Take note however that the PEBC exams are available<br />

to applicants from across Canada and some <strong>of</strong> the requirements<br />

for registration with the <strong>Ontario</strong> <strong>College</strong> <strong>of</strong> <strong>Pharmacists</strong><br />

differ from other provinces. For example, the proposed<br />

deadline for completion <strong>of</strong> the evaluation exam for<br />

those pursuing registration in <strong>Ontario</strong> is <strong>January</strong> 1, 2012. In<br />

other provinces, where the regulation initiative is in much<br />

earlier stages <strong>of</strong> development, the deadline will be extended<br />

Case 1<br />

Member found guilty <strong>of</strong> an <strong>of</strong>fence<br />

relevant to his suitability to practise<br />

Member: Roger Demers<br />

Hearing Date: October 8, 2008<br />

Facts<br />

The <strong>College</strong> commenced an investigation<br />

into the Member’s conduct after<br />

learning that the Member may have<br />

been charged with a criminal <strong>of</strong>fence.<br />

Through subsequent inquiries, the <strong>College</strong><br />

learned that in or about July 2004,<br />

the Member had been charged with<br />

possessing child pornography. Through<br />

his counsel, the Member cooperated<br />

with the <strong>College</strong> concerning the criminal<br />

proceedings.<br />

In an Agreed Statement <strong>of</strong> Facts<br />

with the Crown, the Member admitted<br />

to having used his home computer<br />

to purchase access to three different<br />

websites on three separate occasions in<br />

April 2002, April 2003, and June 2003,<br />

which websites contained child pornography<br />

as defined by the Criminal<br />

Code <strong>of</strong> Canada. He further admitted<br />

to having accessed additional child pornography<br />

websites during the period<br />

March 8, 2000 to July 28, 2004, and<br />

to having in his possession at his home<br />

10 printed images and 34 electronic images<br />

<strong>of</strong> child pornography. The Member<br />

pleaded guilty to the charge <strong>of</strong> possession<br />

<strong>of</strong> child pornography and was<br />

sentenced in criminal court.<br />

The discipline hearing proceeded<br />

by way <strong>of</strong> Agreed Statement <strong>of</strong> Facts<br />

and Joint Submission on Penalty. The<br />

Member admitted that he had committed<br />

acts <strong>of</strong> pr<strong>of</strong>essional misconduct,<br />

namely that he had been found guilty<br />

<strong>of</strong> an <strong>of</strong>fence relevant to his suitability<br />

to practise, and that he had engaged in<br />

conduct or performed an act relevant<br />

to the practice <strong>of</strong> pharmacy that, having<br />

regard to all the circumstances,<br />

would reasonably be regarded by members<br />

<strong>of</strong> the pr<strong>of</strong>ession as disgraceful,<br />

dishonourable or unpr<strong>of</strong>essional.<br />

The Joint Submission on Penalty included<br />

a reprimand, a suspension <strong>of</strong> the<br />

Member’s Certificate <strong>of</strong> Registration<br />

for two months to be served in two<br />

one-month blocks, and costs <strong>of</strong> $4,500<br />

to the <strong>College</strong>. The Joint Submission<br />

also included certain terms, conditions<br />

and limitations on the Member’s Certificate<br />

<strong>of</strong> Registration with respect to<br />

providing current and future employers<br />

with the Discipline Panel’s decision.<br />

Decision and Reasons<br />

The Discipline Panel found this case<br />

to be very challenging when it came<br />

to crafting a penalty. This is a case<br />

that underscores that a pharmacist’s<br />

behaviour outside <strong>of</strong> the place <strong>of</strong> employment<br />

is integrally connected to his<br />

or her pr<strong>of</strong>essional standing in the community<br />

and in the pr<strong>of</strong>ession. The circumstances<br />

<strong>of</strong> the Member’s conduct<br />

are troubling, but the Panel believed<br />

the Member is paying his dues in regards<br />

to the criminal process. In the<br />

pr<strong>of</strong>essional disciplinary context, his<br />

misconduct does not go to his fitness<br />

to practise pharmacy on a technical or<br />

skills based level, but rather his moral<br />

fitness to hold himself out as a pr<strong>of</strong>essional<br />

pharmacist. The Panel could not<br />

simply order course work, order a suspension<br />

and impose a fine as a means<br />

to protect the public, and trust that the<br />

Member will not re<strong>of</strong>fend. Rather, the<br />

Panel’s focus in protection <strong>of</strong> the public<br />

was perhaps even more heightened<br />

than in more typical discipline cases.<br />

The Panel had two problems with<br />

the Joint Submission on Penalty. The<br />

Panel was acutely aware <strong>of</strong> the overwhelming<br />

body <strong>of</strong> case law directing it<br />

to accept Joint Submissions, but did not<br />

feel it could do so in this case without<br />

implementing greater protections for<br />

the public. To the credit <strong>of</strong> the Member,<br />

the <strong>College</strong> and their respective<br />

counsel, the Panel was able to raise its<br />

concerns at the hearing, and the parties<br />

were able to come up with a suitable<br />

additional term to further protect<br />

the public.<br />

The Panel was concerned about<br />

members <strong>of</strong> the public learning, after the<br />

fact, that the Member, as their pharmacist,<br />

may have been counselling their children<br />

on prescriptions given the nature<br />

<strong>of</strong> the Member’s criminal conduct. Accordingly,<br />

the Panel proposed that the<br />

Joint Submission be amended to include<br />

a term that the Member would not counsel<br />

patients under the age <strong>of</strong> 18 without<br />

an adult being present during that counselling,<br />

with the Panel’s preference being<br />

that the other adult be a member <strong>of</strong><br />

the pr<strong>of</strong>essional staff <strong>of</strong> the pharmacy.<br />

When attendance by another pr<strong>of</strong>essional<br />

staff member was not possible, the<br />

Panel expected the Member to pursue<br />

alternative methods, such as counselling<br />

by telephone, or the patient reattending<br />

another day for counselling.<br />

The Panel was also concerned that<br />

the proposed suspension was to be<br />

broken up into two blocks <strong>of</strong> time, to<br />

accommodate some scheduling difficulties<br />

<strong>of</strong> the owner/Designated Manager<br />

<strong>of</strong> the pharmacy in which the Member<br />

was employed. Without being disrespectful<br />

to the pharmacy owner, the<br />

Panel felt that the Discipline Committee<br />

must address the misconduct <strong>of</strong><br />

the Member without getting tied into<br />

pharmacy scheduling demands. The<br />

parties addressed the Panel’s concern,<br />

and came up with alternative dates for<br />

the Member to serve his suspension in<br />

a consistent block <strong>of</strong> time.<br />

Having addressed these areas <strong>of</strong> concern,<br />

the Panel was satisfied with the<br />

remainder <strong>of</strong> the Joint Submission.<br />

Order:<br />

1. A reprimand.<br />

2. Specified terms, conditions and limitations<br />

on the Member’s Certificate<br />

<strong>of</strong> Registration, and, in particular,<br />

that the Member:<br />

a) confirm in writing to the Manager,<br />

Investigations and Resolutions, the<br />

name and address <strong>of</strong> each pharmacy<br />

in which he practises as a<br />

dispensing pharmacist;<br />

b) provide a copy <strong>of</strong> the decision <strong>of</strong><br />

the Discipline Committee from<br />

the Discipline hearing <strong>of</strong> October<br />

8, 2008 (“Decision”) to the<br />

Designated Manager <strong>of</strong> any place<br />

in which he practises, within one<br />

week from the date <strong>of</strong> receipt <strong>of</strong><br />

the Decision;<br />

c) request that any Designated Manager<br />

to whom a copy <strong>of</strong> the Decision<br />

has been provided write to<br />

the Manager, Investigations and<br />

Resolutions, to confirm receipt <strong>of</strong><br />

the Decision; and<br />

d) until June 14, <strong>2009</strong>, refrain from<br />

providing counselling to anyone<br />

under the age <strong>of</strong> 18 years <strong>of</strong> age<br />

unless in the presence <strong>of</strong> another<br />

person 18 years <strong>of</strong> age or older.<br />

3. A suspension <strong>of</strong> the Member’s Certificate<br />

<strong>of</strong> Registration for a period<br />

<strong>of</strong> two months.<br />

4. Costs to the <strong>College</strong> in the amount<br />

<strong>of</strong> $4,500.00.<br />

Reprimand<br />

The Panel had a difficult time reviewing<br />

this case, due to the nature <strong>of</strong> the<br />

misconduct.<br />

The Member is now aware that the<br />

actions he takes in his personal life can<br />

and do have a significant impact, not<br />

only on him but also on the pr<strong>of</strong>ession,<br />

since such actions shake the trust the<br />

public has in their pharmacist. The<br />

Panel appreciated how embarrassing<br />

the experience <strong>of</strong> going through the<br />

criminal process must have been for the<br />

Member, but it wanted the Member to<br />

consider how uncomfortable it must<br />

have been for the Member’s patients to<br />

have learned that someone they trusted<br />

and relied upon, when their children<br />

or they themselves were sick, had engaged<br />

in viewing child pornography.<br />

The Panel hoped that the Member<br />

learned a valuable lesson throughout<br />

this process, and did not expect to see<br />

him before the Discipline Committee<br />

On behalf <strong>of</strong> the <strong>College</strong>, we would<br />

like to thank the pharmacists<br />

who served as preceptors in the<br />

Structured Practical Training (SPT)<br />

program in 2008. By welcoming a<br />

pharmacy student or intern to their<br />

pharmacy team, these preceptors<br />

have continued the tradition<br />

<strong>of</strong> pharmacists sharing time,<br />

experience, and enthusiasm for our<br />

pr<strong>of</strong>ession with a future colleague.<br />

Students and interns continue<br />

to express appreciation to their<br />

preceptors for the encouragement<br />

and learning opportunities that the<br />

preceptors have provided. Many<br />

preceptors described the experience<br />

<strong>of</strong> having a student or intern in their<br />

practice as refreshing, rewarding,<br />

and educational. <strong>Pharmacists</strong><br />

enjoyed discovering what new<br />

graduates are learning, and seeing<br />

where the pr<strong>of</strong>ession is heading.<br />

Conducting medication reviews as<br />

part <strong>of</strong> the Meds Check Program<br />

and running clinic days continue to<br />

be popular activities that interns<br />

incorporate into their rotation to<br />

demonstrate their pharmaceutical<br />

care and communication skills.<br />

2008 HIGHLIGHTS<br />

This year, more than 400 pharmacists<br />

attended one <strong>of</strong> 22 preceptor<br />

workshops across the province.<br />

The one-day workshops were <strong>of</strong>fered<br />

in Burlington, London, Ottawa, Sudbury,<br />

and Toronto. Fifteen Orientation<br />

workshops were held for first-time<br />

preceptors or pharmacists who had last<br />

acted as a preceptor several years ago.<br />

An OCP Registration Advisor and an<br />

experienced preceptor facilitate the<br />

Orientation workshops, in which they<br />

explain the goals <strong>of</strong> the SPT program,<br />

the role and responsibilities <strong>of</strong> a preceptor,<br />

and the expectations for students<br />

and interns, and make pharmacists<br />

aware <strong>of</strong> what skills that they will need<br />

as preceptors. Using videos, role-playing<br />

and guided discussions, the pharmacists<br />

learn how to set expectations,<br />

motivate students and interns, facilitate<br />

practice opportunities, and provide<br />

feedback and assessment. In response<br />

to feedback from preceptors, the Registration<br />

Advisors have made a number<br />

<strong>of</strong> changes to the Orientation workshop.<br />

In <strong>January</strong>, the workshop was<br />

revamped so that the entire afternoon<br />

is now spent working in small groups<br />

to discuss how to guide the student or<br />

intern to complete the activities, and<br />

how the preceptor should review the<br />

student or intern’s activities before they<br />

are submitted to OCP.<br />

The Advanced workshop is <strong>of</strong>fered<br />

to preceptors who last attended<br />

a workshop more than three years<br />

ago and who wish to continue acting<br />

as preceptors. The Advanced workshop<br />

is intended to provide an opportunity<br />

for preceptors to enhance their<br />

teaching and assessment skills and to<br />

share their experiences with other preceptors.<br />

We were pleased to have<br />

Dr. Zubin Austin continue to conduct<br />

the “Conflict Analysis & Resolution”<br />

workshop, in which preceptors are<br />

introduced to the way in which conflict<br />

may arise in the workplace, assess<br />

how to identify their own conflict<br />

management style, and explore how<br />

to develop new methods for addressing<br />

disagreements and negotiating mutually<br />

acceptable solutions. Response<br />

to this workshop has been positive and<br />

we will continue to <strong>of</strong>fer it in <strong>2009</strong>. Dr.<br />

Lionel Laroche continued to receive<br />

a very positive response to his workshop,<br />

“Managing Cultural Differences.”<br />

This workshop provided preceptors<br />

with insight on cultural differences<br />

that may exist between Canadian<br />

and International Pharmacy Graduates<br />

(IPGs) and helped them prepare to<br />

face the challenges and reap the opportunities<br />

<strong>of</strong> being preceptors to a diverse<br />

population <strong>of</strong> students. We look forward<br />

to introducing new workshop<br />

topics in <strong>2009</strong>.<br />

Given the increasing number <strong>of</strong> students<br />

and interns in the SPT program,<br />

we have expanded our network <strong>of</strong> external<br />

pharmacists who review the<br />

SPT activities submitted to OCP. We<br />

thank them for their commitment to<br />

providing coaching and individualized<br />

feedback to students and interns on<br />

their activities in a timely manner.<br />

The SPT assessment forms and activity<br />

documentation forms are available<br />

online. The SPT Studentship<br />

and Internship Manuals continue to be<br />

available online for members. Several<br />

returning preceptors have commented<br />

that they find the current SPT experience<br />

to be much improved and relevant<br />

to current practice.<br />

Once again, a sincere thank you to<br />

our preceptors, facilitators, presenters,<br />

and reviewers for their valuable contribution<br />

to the SPT program and to the<br />

future <strong>of</strong> pharmacy!<br />
