HOW TO COMPLETE AWARDS APPLICATIONS<br />
1. Photocopy application—use one for a rough draft, one to be turned in. It is a good idea to make a copy<br />
of the completed application for your records.<br />
2. Read directions and entire application twice before beginning.<br />
3. Please type or print neatly.<br />
4. Use the member record book, computer, and/or calendars to help you fill out the application.<br />
5. Be brief and to the point. Do not add pages unless indicated. When it is necessary to add a page, be<br />
sure to use 8 1/2” x 11” paper. Do not repeat things several times on the application.<br />
6. Double check for misspelled words, poor grammar, and incomplete sentences.<br />
7. If your parent is the club leader, have another adult leader sign the application and write the letter of<br />
recommendation.<br />
8. Be sure the application has proper signatures. All applications must be filled out by participant as noted<br />
in the <strong>Awards</strong> Handbook.<br />
9. All applications must be postmarked or in the <strong>Oakland</strong> <strong>County</strong> 4-H office by the due date.<br />
10. If called for a personal interview, please dress neatly and do not chew gum, candy, etc.<br />

have been accepted to study at Michigan State University and are planning to attend MSU<br />

This club award is designed to recognize excellent club programming in <strong>Oakland</strong> <strong>County</strong>.<br />
Members are encouraged to participate in Club, <strong>County</strong>, State, and National Events and<br />
International Exchange Programs. <strong>Awards</strong> will be made based on the completeness of the<br />
application. If club only participates in the county level, the awards will reflect that. Then<br />
if the club adds State and National levels later, those portions of the award will be given at<br />

INSTRUCTIONS<br />
About the All Star Award:<br />
This award is an introduction to the <strong>County</strong> 4-H <strong>Awards</strong> Program. The award is presented at the<br />
Annual <strong>Oakland</strong> <strong>County</strong> awards event which is held in October.<br />
The All Star awardees will receive a patch, a commendation letter from his/her county<br />
commissioner, a letter from <strong>County</strong> Executive L. Brooks Patterson, a letter from <strong>Oakland</strong> <strong>County</strong> Farm<br />
Bureau, and an All Star certificate.<br />
1. Selection is made by either the leader or by vote of the club members. Consider such qualities as<br />
willingness to help others, the ability to learn and share skills and the growth of the person as a result<br />
of their previous 4-H experiences.<br />
2. Each club should be represented by two awardees.<br />
3. Eligibility is not dependent upon age or length of enrollment<br />
4. Those members who are ineligible for this All Star Award are: those youth being recognized for<br />
<strong>County</strong> Medals, State <strong>Awards</strong>, Key Participants, “I Dare You” recipients, as well as other state event<br />
winners, or other special honorees who will automatically be invited.<br />

COUNTY MEDALS<br />
WHAT IS A COUNTY MEDAL?<br />
A <strong>County</strong> Medal is a form of recognition awarded to a 4-H member for outstanding project work. You<br />
may receive an award more than once, but will receive only one medal. Each 4-H Member may apply for<br />
two <strong>County</strong> Medals every year. A separate application must be filled out for each one. Applications must<br />
be completely filled out by the 4-H member.<br />
WHAT ARE THE REQUIREMENTS FOR APPLYING FOR COUNTY MEDALS?<br />
A 4-H member must be 11 to 19 years old by January 1 of current year. A member must have completed<br />
one year in the project. <strong>County</strong> Medal forms are due in the 4-H office before August 15.<br />

Community Service is working to satisfy community needs on a volunteer basis. Registered 4-H Clubs<br />
and other volunteers often join in community-based activities to help communities and other groups<br />
accomplish seemingly impossible tasks. 4-H members donate many hours in many different ways to help<br />
make their neighborhoods and communities a better place to live.<br />

THE “I DARE YOU” LEADERSHIP AWARD<br />

BACKGROUND<br />
Mr. William H. Danforth’s book, “I Dare You”, was first published in 1941. It is still considered one of<br />
the most influential books in the general area of personal motivation. Now in its 29th printing, the book is<br />
published and distributed by the American Youth Foundation through it’s “I Dare You” Committee.<br />
Mr. Danforth, founder of the Ralston Purina Company and one of the founders of the American Youth<br />
Foundation, dedicated much of his time and philanthropy to youth work. Through the urging of friends,<br />
he started granting the “I Dare You” <strong>Awards</strong> to outstanding youth in high schools and the 4-H organization<br />
over sixty years ago.<br />
The American Youth Foundation, founded in 1924, is a non-profit organization dedicated to providing<br />
programs for youth. The aim of the foundation is the discovery and enhancement of leadership skills and<br />
the promotion of balanced growth and development.<br />
INVITATIONS<br />
Each year every high school in the Unites States is offered, by letter, the opportunity to nominate two students<br />
in their junior or senior year for recognition. In addition, every county 4-H Extension Educator is<br />
contacted by letter seeking nomination of outstanding 4-H youth for recognition.<br />
Many schools and counties have participated every year. All are invited regardless of past participation.<br />
THE AWARD<br />
The “I Dare You” Leadership Award is not a contest or a competition of any kind. Each high school principal<br />
or headmaster and each <strong>County</strong> 4-H Extension Educator is asked to select (or have a staff or committee<br />
select) the nominees. The Award is offered as recognition of excellence in character and wellbalanced<br />
personal development, as well as demonstrated qualities of constructive leadership. The application<br />
should be completed by a leader (other than parent) or a teacher.<br />
The selected applicants will receive a copy of the book “I Dare You”, a personalized certificate, and are<br />
eligible for a partial scholarship to attend the American Youth Foundation National Leadership Conference<br />
at Camp Miniwanca, MI. This Award is presented at the All Star Review.

The Friend of 4-H Award is an opportunity for 4-H clubs to recognize new 4-H individuals and<br />

This award has been created to encourage clubs to elevate their programs to a higher level of achievement. The criteria<br />
for winning this award is listed below in various categories of programming. The winner of this award will receive a<br />
framed certificate and a rosette presented at the <strong>Oakland</strong> <strong>County</strong> All Star Review.<br />
CLUB Level Criteria To be considered for this award, as a minimum the following activities in this level are required.<br />
