Chris Knaggs – Housing Manager from Lambeth Housing has not been able to establish who owns these properties and therefore cannot comment why the flats are boarded up. Chris will pass the information to the empty homes officer. Q. Manny Oraka asked why there are so many rough sleepers people in Lambeth. A. Health Action Plans (HAP) Irene Bezzina, Pauline Pepperrell and Gillian Bretherton presented slide show overheads about what HAPs are, how they can help you and how to get one. A Health Action Plan writes what needs to be done to help the person to stay healthy. It also writes what support the person may need to do this. A health facilitator helps you make your HAP. A Health facilitator can be a friend or a family member or even yourself. The targets of ‘Valuing People’ are that all people with a learning disability should have a Health Action Plan by June 2005. There is to be a HAP steering group set up soon, so decisions can be made on how to let people know about HAP. GP’s have been informed and the PCT have training schemes for GP’s in January and February. PCP and HAP do have links but it was stated that each has a purpose within its own right. One could work in tandem with the other. Register services were highlighted as a good way to let people know about HAP and PCP. Lambeth Learning Disability Partnership Board 7
