– Original through channels to Support Services
– “Follow-Up” (yellow) copy to station file
– “File” (pink) copy will be returned as proof of receipt
– Upon work completion, fill out bottom of “Follow-Up”
(yellow) copy – “work completed” - and return it to
Support Services
