Ramona Mack, age 40 <strong>of</strong> Winfield, plead guilty on July 20, <strong>2009</strong> to one (1) count <strong>of</strong> identity <strong>the</strong>ft and<br />

one (1) count <strong>of</strong> taking SSI benefits to which she wasn't entitled, while five o<strong>the</strong>r charges were<br />

dismissed as part <strong>of</strong> <strong>the</strong> plea deal. The OIC Fraud Unit assisted U. S. Postal Inspectors with this<br />

identity <strong>the</strong>ft case. Ms. Mack used several alias variations <strong>of</strong> her name and twenty two (22) or more<br />

different social security numbers. Suspect <strong>the</strong>n used <strong>the</strong>se various names and social security numbers<br />

to purchase a home, vehicles, insurance policies, and to secure employment. As part <strong>of</strong> <strong>the</strong> plea<br />

agreement, Ms. Mack agreed to repay over $372,000 in restitution to <strong>the</strong> affected entities.<br />

Spring Fisher, age 37 and Joseph Fisher age unknown, husband and wife, both <strong>of</strong> Charleston, plead<br />

guilty on December 27, <strong>2009</strong> to <strong>the</strong> reduced count <strong>of</strong> Misdemeanor Offenses <strong>of</strong> Conspiracy to Commit<br />

a Misdemeanor and entered into a pretrial diversion program. Both had originally been indicted on<br />

one felony count each <strong>of</strong> uttering, forgery and 2 felony counts <strong>of</strong> conspiracy. Joseph Fisher presented<br />

a forged document/insurance certificate to a Kanawha County Magistrate Court in exchange for <strong>the</strong><br />

dismissal <strong>of</strong> a traffic citation and did so on July 03, 2006. Additionally, Mr. Fisher <strong>the</strong>n sent <strong>the</strong> same<br />

forged insurance certificate through <strong>the</strong> US Postal Service to <strong>the</strong> WV Division <strong>of</strong> Motor Vehicles as<br />

documented pro<strong>of</strong> <strong>of</strong> insurance. Both Fisher’s conspired to seek out a person to fraudulently compile<br />

an insurance certificate to make it appear valid prior to <strong>the</strong> submitting <strong>of</strong> <strong>the</strong> forged<br />

document/insurance certificate to <strong>the</strong> Kanawha County Magistrate and <strong>the</strong> WV Division <strong>of</strong> Motor<br />

Vehicles.<br />

Lucille Fowler, Phillip Justice, Laura Frazier, Anthony Lambert, Mat<strong>the</strong>w Fruia, James Fruia,<br />

Kimberly Fruia, Donald Justice, Phillip Leonard, Lidge Marcum and Mary Marcum , James<br />

Bailey, Amy Hunt, did knowingly and willfully and with intent to defraud, submit materially false<br />

insurance claims with State Farm Fire and Casualty, Nationwide <strong>Insurance</strong> and Sentry <strong>Insurance</strong> from<br />

a period beginning March 5, 1999 through February 9, 2007 for reported intentionally staged accidents<br />

and o<strong>the</strong>r similar incidents. All <strong>of</strong> <strong>the</strong> participants that took part in <strong>the</strong>se fraudulent claims received<br />

settlement checks for bodily injury or medical services and in some cases received both. Unless paid<br />

directly by <strong>the</strong> provider, Lucille Fowler and <strong>the</strong> o<strong>the</strong>rs did not reimburse medical providers as agreed<br />

to in <strong>the</strong>ir release forms. The investigation and prosecution into this staged auto crash ring covered<br />

approximately three (3) years. All participants ei<strong>the</strong>r plead or were found guilty in 2008 and <strong>2009</strong>. The<br />

“leader” <strong>of</strong> <strong>the</strong> group, Lucille Fowler was sentenced to one (1) to three (3) years in <strong>the</strong> state<br />

penitentiary and ordered to pay all court costs within one (1) year from her release from jail.<br />

Sharon Ann Glassburn, age 63 <strong>of</strong> Charleston plead guilty on January 23, <strong>2009</strong> to creating a<br />

fraudulent certificate <strong>of</strong> insurance for her son which was presented to <strong>the</strong> Kanawha County Magistrate<br />

court as well as faxed to <strong>the</strong> WV Department <strong>of</strong> Motor Vehicles. Ms. Glassburn was sentenced to two<br />

(2) years probation and ordered to pay associated costs for her probation, as well as all court costs.<br />

Lisa K. Hall, age 28, <strong>of</strong> Man plead guilty to on February 26, <strong>2009</strong> to one (1) count <strong>of</strong> Accessory to<br />

Forging False Certificates <strong>of</strong> <strong>Insurance</strong> and was fined $100 plus court costs. Ms. Hall had conspired<br />

with Cindy B. Hall, age unknown, who also plead guilty to “Forgery <strong>of</strong> an <strong>Insurance</strong> Certificate, to<br />

provide a false certificate <strong>of</strong> insurance and faxed <strong>the</strong> same to <strong>the</strong> WV Department <strong>of</strong> Motor Vehicles<br />

for Lisa K. Hall. Cindy B. Hall was fined $50 plus court costs for her role.<br />
