5. <strong>Management</strong> <strong>of</strong> Physical Injuries<br />

Post traumatic vacc<strong>in</strong>ati<strong>on</strong> with tetanus toxoid<br />

• Clean abrasi<strong>on</strong>s and superficial lacerati<strong>on</strong>s with antiseptic and either dress<br />

or pa<strong>in</strong>t with t<strong>in</strong>cture <strong>of</strong> iod<strong>in</strong>e, <strong>in</strong>clud<strong>in</strong>g m<strong>in</strong>or <strong>in</strong>juries to the vulva and<br />

per<strong>in</strong>eum;<br />

• If stitch<strong>in</strong>g is required, stitch under local anaesthesia. If the survivor’s level<br />

<strong>of</strong> anxiety does not permit, c<strong>on</strong>sider sedati<strong>on</strong> or general anaesthesia;<br />

• High vag<strong>in</strong>al vault, anal and oral tears and 3rd/4th degree per<strong>in</strong>eal <strong>in</strong>juries<br />

should be assessed under general anaesthesia by a gynaecologist or other<br />

qualified pers<strong>on</strong>nel and repaired accord<strong>in</strong>gly;<br />

• In cases <strong>of</strong> c<strong>on</strong>firmed or suspected perforati<strong>on</strong>, laparatomy should be<br />

performed and any <strong>in</strong>tra-abdom<strong>in</strong>al <strong>in</strong>juries repaired <strong>in</strong> c<strong>on</strong>sultati<strong>on</strong> with<br />

a general surge<strong>on</strong>;<br />

• Provide analgesics to relieve the survivor <strong>of</strong> physical pa<strong>in</strong>;<br />

Where any physical <strong>in</strong>juries result <strong>in</strong> breach <strong>of</strong> the sk<strong>in</strong> and mucous membranes,<br />

immunize with 0.5mls <strong>of</strong> tetanus toxoid accord<strong>in</strong>g to the revised schedule dem<strong>on</strong>strated<br />

<strong>in</strong> the table below.<br />

Tetanus toxoid should be given to all survivors <strong>of</strong> sexual violence (all sexes and all<br />

ages) if there are any physical <strong>in</strong>juries <strong>of</strong> the sk<strong>in</strong> and/or mucous membranes.<br />
