REVISED FORMAT OF COVERING LETTER AND
CERTIFICATION FOR DOCUMENTS IN PHOTOCOPY
DATE :
22 FEBRUARY 2OO7
Effective 15 March 2007, all manning agencies, shipping companies and
seafarer's organization/association submitting applications for issuance of
certificate of Proficiency at the National Assessment Center (NAC) shall use the
revised covering letters (For Existing and New Entrant Seafarers) and
certification for documents in photocopy.
Attached are Annexes A, B, and C for guidance.
