This in<strong>for</strong>mation is confidential <strong>and</strong> protected by attorney-client privilege.<br />

Purpose: To obtain the facts necessary <strong>for</strong> an <strong>immigration</strong> expert to determine current <strong>immigration</strong> status,<br />

possible <strong>immigration</strong> relief, <strong>and</strong> <strong>immigration</strong> consequences of a conviction <strong>and</strong>. For more in<strong>for</strong>mation on<br />

Documents: Photocopy any <strong>immigration</strong> documents/passport.<br />

Criminal History: Rap sheets <strong>and</strong> possible current plea-bargain offenses needed be<strong>for</strong>e calling.<br />

Note: While completing this questionnaire, on a separate sheet of paper create one chronology showing<br />

dates of criminal acts <strong>and</strong> convictions as well as the <strong>immigration</strong> events discussed in the questionnaire.<br />
