Busy mom Carmen Gonzalez chose our Diamond<br />

Care Medical Home for her son Aaron (shown here<br />

many <strong>San</strong> Diegans, Carmen recently lost her job<br />
and is looking for work. Aaron’s dad has steady<br />
work as a mechanic, but his job doesn’t provide health<br />
insurance for the family. Thankfully, Carmen<br />
found our health center so her active preschooler<br />
gets all <strong>of</strong> the healthcare services he needs!

Breast Pump Loan Program<br />
August was National Breast Feeding month and our partners showed<br />
their support in a big way. Thanks to the <strong>San</strong> <strong>Diego</strong> Breastfeeding<br />
Coalition and Rest Haven Children’s Fund, <strong>Family</strong> <strong>Health</strong> <strong>Centers</strong><br />
<strong>of</strong> <strong>San</strong> <strong>Diego</strong> now has a new breast pump loan program! Through the<br />
program, we can loan 10 multi-user electric pumps to new moms.<br />
Breast milk is best for babies, and now we can better support new<br />
breast feeding moms who can not afford to purchase the pumps<br />
on their own.<br />

Recognition<br />
Designated Primary Care Medical Home<br />
Since our founding in 1970, <strong>Family</strong> <strong>Health</strong> <strong>Centers</strong> <strong>of</strong> <strong>San</strong> <strong>Diego</strong> has<br />
been dedicated to providing coordinated care to our patients. This<br />
summer, The Joint Commission formally designated <strong>Family</strong> <strong>Health</strong><br />
<strong>Centers</strong> <strong>of</strong> <strong>San</strong> <strong>Diego</strong> as a Primary Care Medical Home (PCMH).<br />
The PCMH criteria demonstrates an organization’s commitment to<br />
ensuring their patients receive timely and well-coordinated<br />
treatment. The criteria promotes increased patient satisfaction,<br />
improved patient outcomes and reduced overall costs to the health<br />
care system. The PCMH model exemplifies the new federal health<br />
care reform efforts to improve health outcomes and insure the<br />
coordination, quality and efficiency <strong>of</strong> health care services.<br />
Physician Assistant Lori Saunders holds Leondrus White,<br />
who came to the health center with his mom Leantonette<br />
“LeeLee” White. LeeLee also received case management<br />
and support services through our Black Infant <strong>Health</strong><br />
Program during her pregnancy - just one part <strong>of</strong> the<br />
comprehensive service system that makes up our Primary<br />
Care Medical Home system <strong>of</strong> care.<br />
<strong>Family</strong> <strong>Health</strong> <strong>Centers</strong> <strong>of</strong> <strong>San</strong> <strong>Diego</strong> is honored to receive this<br />
external validation <strong>of</strong> our care delivery system. Our dedicated<br />
clinical and support teams work together to coordinate high<br />

Rice Elementary in Chula Vista<br />
Great news! We just received a federal school based health<br />
center grant to establish a health center on the campus <strong>of</strong> Rice<br />
Elementary in Chula Vista! The new health center will open<br />
in 2012 through a partnership with the Chula Vista Elementary<br />
School District. It will provide healthcare services to Rice<br />
students, helping them stay healthy and in school, as well as<br />
their families and local community.<br />
Everyone is excited about the new health center at Rice Elementary in Chula Vista!<br />
Watch City Heights Grow<br />
We are excited about the progress <strong>of</strong> our new <strong>Family</strong> <strong>Health</strong><br />
<strong>Centers</strong> <strong>of</strong> <strong>San</strong> <strong>Diego</strong>’s City Heights <strong>Family</strong> <strong>Health</strong> Center. Each day <strong>of</strong><br />
construction we move closer to our new and expanded health<br />

A very special gift that will secure your<br />
family’s future is a plan that describes<br />
your intentions for handling property<br />
and responsibilities. Studies show that<br />
more than 75% <strong>of</strong> the people who can<br />
benefit from estate planning never do<br />
because they procrastinate.<br />

and who will be named to care for<br />

Beneficiary: Term used to describe a<br />
person who benefits from a trust, will,<br />
IRA, or other assets.<br />
Estate: All property you own, in any<br />
form, that will someday pass to loved<br />
ones and/or charitable organizations.<br />
Trust: A structure that outlines ownership<br />
and management <strong>of</strong> your property.<br />
Trustee: The individual or entity charged<br />
with managing trust assets on a day-today<br />
basis. In most cases, this is you.<br />
Successor Trustee: The individual<br />
or entity charged with taking over<br />
management <strong>of</strong> trust assets if the<br />
trustee becomes incapacitated or dies.<br />
Will: Instructions to guide distribution<br />
<strong>of</strong> your property or estate. When used<br />
in tandem with a revocable living trust,<br />
it normally directs the distribution <strong>of</strong><br />
personal property.<br />
Executor: Person or entity chosen<br />
to carry out the terms <strong>of</strong> your will;<br />
sometimes referred to as a “personal<br />
representative.”<br />
Power <strong>of</strong> Attorney: A legal document<br />
empowering others to take action on<br />
your behalf. Durable Powers <strong>of</strong> Attorney<br />
are used for the purpose <strong>of</strong> empowering<br />
others if you become incapacitated.<br />
Advance <strong>Health</strong> Care Directive:<br />
A document that declares your wishes<br />
outlining medical care and life support.<br />
This document is also used to appoint<br />
someone to make medical decisions on<br />
your behalf if you are unable to make<br />
these decisions for yourself.<br />
Please always consult with your<br />
attorney or pr<strong>of</strong>essional advisor as you<br />
take steps to plan your estate now. For<br />
