DRIVER ADDRESS Transcribe the driver's address as shown on the driver's license unless it is<br />

incorrect. Check the “New address?” checkbox if their current address is different from the license.<br />

Enter the street address with the apartment number if any, or the rural route number, and the city,<br />

state, and zip code. Record addresses of unlicensed persons or pedestrians as completely as<br />

possible.<br />

GENDER Record the gender for all vehicle occupants, pedestrian types, train crew, and injured<br />

train passengers. If the gender is unknown, record a ‘U’. Valid codes are: ‘M’, ‘F’, and ‘U’.<br />

AGE Record the age for all vehicle occupants, pedestrian types, train crew, and injured train<br />

passengers.<br />

Valid Age codes are:<br />

NN – Birth to 24 hours<br />

NB – 1 to 6 days old<br />

BB – 7 days to 364 days old<br />

01-98 – exact age in years<br />

99 – Ninety-nine years old and older<br />

00 - Unknown<br />

SAFETY EQUIPMENT USE This is a mandatory<br />

field for all drivers, passengers, and pedestrians.<br />

Do NOT use ‘Y’ meaning Yes. If the Safety<br />

Equipment is not known, record a ‘U’. All Safety<br />

Equipment codes are shown on the code sheet<br />

(form 855). Note: Booster Seats are normally used<br />

between the ages of 4-8 where the child is<br />

between 40 and 80 lbs.<br />

Airbag Codes: Use the ‘Airbag’ codes that<br />

affect only the seat position (if occupied) that<br />

applies to the airbag’s deployment.<br />

In the example, the only occupants<br />

requiring airbag codes would be the<br />

driver (1) and “shotgun” position (3).<br />

If a side airbag deployed for another<br />

seat position, code it accordingly.<br />
