ago to raise money in an effort to reduce the incidence of pediatric liver
disease through research, awareness, education, and support. Proceeds of
money raised will benefit the Mount Sinai Pediatric Liver Disease Program.

1. The Kellner Family Pediatric Liver Disease Foundation Raffle Prize will be a Rolex Watch.
2. No more than 150 tickets will be sold. Any ticket forms and money turned in after the closure
of the raffle will be returned to the buyer. All ticket forms will be recorded in the order
in which they are received. Any checks or purchases received after the 150 tickets are sold
will be rejected and returned to the buyer.
3. Based upon 150 certificates being sold, each certificate has a 1-in-150 chance of
winning the Grand Prize.
4. The purchase price of 1 raffle ticket is $100.00. Payment will be accepted in the form
of cash, checks, or money orders. All payments made by check or money order should
be made be made payable to “The Kellner Family Pediatric Liver Disease Foundation.”
Upon receipt of either foirm of payment, ticket(s) will be mailed within 15 working days
if not given directly to the entrant. If entrant’s check is dishonored by the bank, the entry
certificate will be declared invalid unless and until full payment is made prior to the
drawing. Any dishonored check will be considered an invalid entry and the entrant will
be notified. All ticket purchases are final and nonrefundable.
5. The Kellner Family Pediatric Liver Disease Foundation drawing will be held on
June 12, 2007. The purchaser(s) of the winning ticket need not be present to win.
6. More than 1 name may be stated on the certificate registration form. The winner(s)
must provide a Social Security number(s). Delivery of the prize will be made solely
to the entrant. It is the sole responsibility of the entrants in a multiple party entry to
allocate the prize among themselves.
7. Purchasers must be 18 years of age or older.
8. Names of the winner(s) will be available to the media and all raffle participants via
The Kellner Family Pediatric Liver Disease Foundation Web site for a minimum of
30 days after the end of the raffle.
9. Winners will assume and be responsible for all licensing, registration, and other fees,
as well as local, state and federal taxes, including without limitation all applicable sales, use,
luxury, income and special taxes. The Kellner Family Pediatric Liver Disease Foundation
DISCLAIMS ALL EXPRESS OR IMPLIED WARRANTIES, INCLUDING WARRANTIES OF
MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE REGARDING THE
ROLEX RAFFLE.
10. The decision of The Kellner Family Pediatric Liver Disease Foundation shall be final on
all matters concerning the raffle.
11. The purchase of tickets for this raffle is void where prohibited by law.
