The Rehabilitation Unit, commonly<br />

known as <strong>Ward</strong> <strong>18</strong>, is a 23 bedded<br />

unit.<br />

It is located in the <strong>James</strong> <strong>Paget</strong><br />

<strong>University</strong> Hospital building and is<br />

easily accessible.<br />

Our aim is to provide:<br />

• A safe environment<br />

• Open communication<br />

• A friendly, welcoming atmosphere<br />

• Privacy and dignity for patients<br />

• Equality to all<br />

To achieve our aim we work with<br />

approachable multidisciplinary team<br />

members, we encourage social<br />

interactions, and maintain a clear and<br />

tidy environment.<br />

On admission to the Rehabilitation<br />

Unit, an individual care plan will be<br />

established for you to meet your<br />

treatment goals and the rehabilitation<br />

team will review this weekly at the<br />

multidisciplinary team meeting.<br />

The rehabilitation team includes:<br />

• Doctors<br />

• <strong>Ward</strong> sisters<br />

• Qualified nursing staff and<br />

rehabilitation assistants<br />

• Discharge co-ordinator<br />

• Case manager<br />

• Physiotherapist<br />

• Occupational therapist<br />

• Therapy assistant practitioner<br />

With input from other specialities as<br />

needed such as:<br />

• Speech and language therapy<br />

• Specialist nurses<br />

• Dietician<br />

Social support is available through<br />

Social Services. A named social worker<br />

will be allocated if social support is<br />

required.<br />

What to expect while you<br />

are on the ward<br />

The Rehabilitation Unit’s aim is to<br />

support patients in achieving as much<br />

independence as possible in their<br />

activities of daily living.<br />

Active patient participation is an<br />

essential part of the treatment process.<br />

Trained staff are available at all times<br />

to enable patients to participate in all<br />

activities of daily living, such as<br />

washing and dressing (with or without<br />

assistance), practising walking and<br />

kitchen tasks, such as making a hot<br />

drink, and helping to do exercises as<br />

advised by the physiotherapist.<br />

The ward case manager will contact<br />

you in the early days of your admission<br />

to discuss your home environment and<br />

discharge plans.<br />

The multidisciplinary team on the unit<br />

work together to manage and review<br />

patients’ care plans while patients are<br />

on the ward. The team co-ordinates<br />

discharge planning with the provision<br />

of ongoing support after discharge,<br />

through community services, for a safe<br />

and timely discharge.
