James Paget University Hospitals NHS Foundation Trust
The Rehabilitation Unit, commonly
known as Ward 18, is a 23 bedded
unit.
It is located in the James Paget
University Hospital building and is
easily accessible.
Our aim is to provide:
• A safe environment
• Open communication
• A friendly, welcoming atmosphere
• Privacy and dignity for patients
• Equality to all
To achieve our aim we work with
approachable multidisciplinary team
members, we encourage social
interactions, and maintain a clear and
tidy environment.
On admission to the Rehabilitation
Unit, an individual care plan will be
established for you to meet your
treatment goals and the rehabilitation
team will review this weekly at the
multidisciplinary team meeting.
The rehabilitation team includes:
• Doctors
• Ward sisters
• Qualified nursing staff and
rehabilitation assistants
• Discharge co-ordinator
• Case manager
• Physiotherapist
• Occupational therapist
• Therapy assistant practitioner
With input from other specialities as
needed such as:
• Speech and language therapy
• Specialist nurses
• Dietician
Social support is available through
Social Services. A named social worker
will be allocated if social support is
required.
What to expect while you
are on the ward
The Rehabilitation Unit’s aim is to
support patients in achieving as much
independence as possible in their
activities of daily living.
Active patient participation is an
essential part of the treatment process.
Trained staff are available at all times
to enable patients to participate in all
activities of daily living, such as
washing and dressing (with or without
assistance), practising walking and
kitchen tasks, such as making a hot
drink, and helping to do exercises as
advised by the physiotherapist.
The ward case manager will contact
you in the early days of your admission
to discuss your home environment and
discharge plans.
The multidisciplinary team on the unit
work together to manage and review
patients’ care plans while patients are
on the ward. The team co-ordinates
discharge planning with the provision
of ongoing support after discharge,
through community services, for a safe
and timely discharge.
