1. Where do I find out the model and serial numbers of the camera I have<br />
purchased?<br />
This is located on the base of the camera and the rear of the carton your Lumix<br />
camera was shipped in.<br />
2. What is a ‘full’ purchase receipt?<br />
You must include a copy of your full purchase receipt and not a deposit<br />
receipt. Please do not send the original.<br />
3. Return Address<br />
Please write your return address clearly on the back of the envelope. This will<br />
enable underpaid/undelivered items to be returned and claims re-submitted.<br />
4. How do I send my claim form to Panasonic?<br />
Place this cashback claim form and a copy of your full purchase receipt<br />
together with a copy of the BARCODE into an envelope, attach a stamp and<br />
post to: Panasonic Lumix TZ20 <strong>Cashback</strong> Promotion, HWA Ltd,<br />
3 Ilex House, 94 Holly Road, Twickenham, TW1 4HF.<br />

1 To qualify for cashback, you must purchase one of the<br />
following new (not second-hand) products: DMC-TZ20EB-A,<br />
DMC-TZ20EB-K, DMC-TZ20EB-R and DMC-TZ20EB-S from a<br />
participating retailer during the promotional period.<br />
2. Promotion Period: Purchases between 15th June 2011 and<br />
24th July 2011 qualify.<br />
3. £30 cashback will be given for purchases made in the UK,<br />
and €35 for purchases made in ROI.<br />
4. To apply for your cashback, please clearly complete your<br />
details above and include a COPY of your full purchase<br />
receipt (not deposit receipt) and a COPY of the barcode<br />
from the carton along with your claim, by 31st August<br />
2011 (unfortunately, receipt copies cannot be returned<br />
so please retain your original). You must send a copy of<br />
your full purchase receipt and not a deposit receipt.<br />
However if you have applied for IFC or any other credit<br />
agreement with your purchasing retailer, and do not have<br />
a full till receipt, then please send a copy of both your<br />
deposit receipt and credit agreement.<br />
5. Entries, with required documentation must be sent to:<br />

House, 94 Holly Road, Twickenham, TW1 4HF.<br />
6. Panasonic does not accept any responsibility for<br />
misdirected, delayed or lost claims, incomplete or illegible<br />
claim forms or claims damaged in the post. <strong>Claim</strong>s by fax,<br />
telephone or e-mail will not be accepted. Only claims made<br />
using this original claim form will be accepted. Illegible,<br />
incomplete or altered claim forms will be deemed void, as<br />
will claim forms not completed in accordance with these<br />
terms and conditions.<br />
7. Upon receipt we will process your cashback claim and send<br />
the cheque made out to the name provided on this claim<br />
form. If you have not received your cashback cheque 45<br />
days after posting your claim form, please contact the<br />
Lumix <strong>Cashback</strong> Helpline on Tel: 0800 072 7649 (Monday to<br />
Friday 9:00 - 17:00) or e-mail: cashback@FOTF.co.uk<br />
8. This is a Panasonic promotion, run in conjunction with<br />
