• Regular and student conference registration includes admission to conference, proceedings, tutorials, workshops,<br />

coffee breaks, lunches on May 18 (Wednesday), May 19 (Thursday) and May 20 (Friday), the reception on<br />

Wednesday night, and the banquet on Thursday night. The workshop proceedings and the tutorial notes must be<br />

purchased separately (Additional banquet tickets, reception tickets, and proceedings can be purchased on-site.)<br />

• Confirmation letter or email will be mailed within 10 days of registration received via fax.<br />

• Cancellation policy:<br />

− Alterations or cancellations of reservations should be addressed to the contact below via fax by May 10 th .<br />

− <strong>Registration</strong> fees will be refunded in full, except for a handling charge of 5500 JPY, in case that can<br />

cellation is received by Dr. Kawasaki by May 10th 2005. Thereafter no refund will be issued.<br />

• In order to qualify the early registration rates, you are requested to fax us your registration by filling<br />

10th, 2005.<br />

• For full-time students, please send us the proof of identity via fax together with your registration<br />
