8 Your Physical Health Community Care does not manage your physical health care. Your physical health plan, or Physical Health Managed Care Organization, takes care of your physical (medical) needs. Your physical health plan is either Keystone Mercy Health Plan, UnitedHealthcare Community Plan, Health Partners of Philadelphia, Inc., ConventryCares from HealthAmerica, or Aetna Better Health. Your Primary Care Physician (PCP) coordinates your medical needs. You will be using the services of your physical health plan when you: • Are sick. • Need a check-up. • Need to get a prescription filled. (This does not apply to all medications, for example, methadone and buprenorphine. Please contact your Primary Care Physician (PCP) with questions, or call Community Care). • Need ambulance transportation. Remember, no matter which physical health plan you choose, you will be a Member of Community Care as long as you are enrolled in HealthChoices. Customer Services 1-866-622-4228
