Your Physical <strong>Health</strong><br />

<strong>Community</strong> <strong>Care</strong> does not manage your physical health care. Your<br />

physical health plan, or Physical <strong>Health</strong> Managed <strong>Care</strong> Organization,<br />

takes care of your physical (medical) needs.<br />

Your physical health plan is either Keystone Mercy <strong>Health</strong> Plan,<br />

United<strong>Health</strong>care <strong>Community</strong> Plan, <strong>Health</strong> Partners of Philadelphia,<br />

Inc., Conventry<strong>Care</strong>s from <strong>Health</strong>America, or Aetna Better <strong>Health</strong>.<br />

Your Primary <strong>Care</strong> Physician (PCP) coordinates your medical needs.<br />

You will be using the services of your physical health plan when you:<br />

• Are sick.<br />

• Need a check-up.<br />

• Need to get a prescription filled. (This does not apply to all<br />

medications, for example, methadone and buprenorphine. Please<br />

contact your Primary <strong>Care</strong> Physician (PCP) with questions, or call<br />

<strong>Community</strong> <strong>Care</strong>).<br />

• Need ambulance transportation.<br />

Remember, no matter which physical health plan you choose, you<br />

will be a Member of <strong>Community</strong> <strong>Care</strong> as long as you are enrolled in<br />

<strong>Health</strong>Choices.<br />
