What is a <strong>SBHC</strong>?<br />
School-based health centers (<strong>SBHC</strong>s) are one of the<br />
ways communities address the unmet health needs of<br />
young children and adolescents in the school setting<br />
during school hours.<br />
<strong>Kaleida</strong> <strong>Health</strong> school-based health centers provide<br />
students in nine Buffalo Public Schools with ageappropriate,<br />
culturally-sensitive primary health care<br />
services.<br />
A nurse practitioner works in partnership with a<br />
<strong>Kaleida</strong> <strong>Health</strong> physician from Women & Children’s<br />
Hospital of Buffalo, as well as the child’s primary<br />
care physician, to provide comprehensive care that<br />
promotes wellness and healthy lifestyle habits for<br />
children, youth and families.<br />
How much does it cost?<br />
Children and adolescents are not charged for these<br />
services. <strong>Health</strong> services are made possible through<br />
support provided by the New York State Department<br />
of <strong>Health</strong>.<br />
How do you sign your child up?<br />
Parents must fill out the enrollment package, which is<br />
available at the school-based health center.<br />
Do you need an appointment?<br />
Walk-ins and appointments are welcome.<br />
What should you do if your child needs health<br />
services after school hours?<br />
Contact your primary care provider for after-hour<br />
emergencies, or call Women & Children’s Hospital<br />
of Buffalo at (716) 878-7000 and ask to speak to the<br />
general pediatric physician on-call.<br />

Important?<br />
They improve the mental and physical health of<br />
students to help them achieve educational success,<br />
which can help them become successful adults.<br />

• Provide counseling and support to students<br />
• Increase chances of achieving educational success<br />
• Provide a safe place for adolescents to talk about<br />

• Improve attendance rate by providing access to<br />
health services on-site<br />
• Help prevent illnesses that may lead to expensive<br />
health care services<br />
• Detect illness to avoid emergency room visits<br />
• Educate students about the importance of good<br />
health care and healthy behaviors<br />
• Respond to emotional needs of students, staff and<br />
parents in times of school-wide crisis<br />
• Reduce the incidence of risk behaviors that<br />
contribute to teen pregnancy<br />

working papers physicals)<br />
• Diagnosis and treatment of acute and chronic<br />

• First Aid (treatment of minor injuries)<br />

• Referral to specialists for services not provided at<br />
the <strong>SBHC</strong><br />

• Behavioral health counseling<br />
• Age-appropriate reproductive health services,<br />
including abstinence counseling, education, exams<br />
