STUDENT ENROLLMENT LIABILITY CONTRACT & TUITION AGREEMENT
This form is being used to register students for The Phoenix Ranch School and is to be completed by the student’s parent or legal guardian. All terms and

AGREEMENT-Both parents and or legal guardian, having the financial responsibility for the minor child, must sign this agreement.
I/We understand that The Phoenix Ranch School is not a special needs school and therefore cannot provide services to accommodate a “special needs” student. I represent that my
child is physically, emotionally, socially, and cognitively able to participate at the required grade level. I represent that my child or ward will participate in all school activities without
infringing upon the rights of others (fighting, stealing, abusive language or behavior), and adhere to The Phoenix Ranch School Behavior Contract enclosed.
I/We will be financially responsible for the tuition and registered programs. In the event that I become delinquent in my financial obligation or need to withdraw my child for any reason,
I know that I will be held responsible for the terms and conditions set forth in this contract. I also understand that tuition does not include the Administrative Fees that must be paid for
with this contract unless other arrangements are made with administration.
I/We understand that field trips will be scheduled on a school day in an effort to expand a child’s learning environment. I understand that I will be required to sign a parent permission
slip in order for my child to participate in the trip. If I decide that my child not participate, I understand that there will be no other schooling provided for that day. Therefore, as the
parent/guardian of that child, I understand that I will be required to make arrangements for the child’s care off school premises. I further understand that there will be no refund or credit
given for a day when a school field trip is scheduled, and my child does not participate in the field trip.
I/We understand that The Phoenix Ranch School shall have the unilateral right to cancel this agreement if payment is not received by date due.
I/We also understand that I/we do not have the right to alter forms, contracts or any written agreement sponsored by The Phoenix Ranch School and Camp.
I/We understand that The Phoenix Ranch School shall have the unilateral right to cancel this agreement at any time and refund any unearned tuition.
I/We have read The Phoenix Ranch School Student Enrollment Liability Contract & Tuition Agreement and I/we must be in compliance with and agree to its stated terms.

General Information
1. Enrollment Agreement and Forms – California State Law requires that forms completed and signed by the student’s parents or guardians must be received prior to
attendance at the Phoenix Ranch School. Forms include, but are not limited to the Physician’s Statement, Identification and Emergency Information, Authorization for Consent
to Treatment of a Minor, Pre-Admission Health History, Personal Rights Form, Student Behavior Contract, and Enrollment Liability Contract and Tuition Agreement.
2. Admittance to School – A student shall not be admitted to school until all of the school registration requirements (fee and forms) have been met and received by the
Phoenix Ranch School administrative offices.
3. Hours of Operation –The Phoenix Ranch School campus opens at 6:30 A.M. and closes at 6:30 P.M., Monday through Friday, with the exception of school holidays. The
Main Office is opened from 7:30 A.M. to 5:00 P.M.
4. Holidays – The school is closed on all national holidays as well as the day after Thanksgiving. The school may choose to close the day before and/or after any holiday that
falls on Tuesday or Thursday. (See the holiday schedule in the Phoenix Ranch Calendar and Handbook.)
5. Vacations and Absences – Payment is required through any absence for enrollment to be continued. Preschool tuition reflects a vacation credit; therefore, regular
payments must be received during all vacations or absences.
6. Late to School (Elementary / Middle School students) – When arriving after 8:45 A.M., all students must be checked in through the Main Office.
7. Late to School (Preschool) – When arriving after 8:45 A.M., all students must sign in their classroom and if the class is in another areas, Parent must then bring the child
to their teacher to acknowledge that the child is present.
8. Illness – Sick children are not admitted to school. Alternative arrangements must be made in case your child becomes ill.
9. Medicine - Medicine will not be administered at school without a doctor’s prescription. A “Request to Administer Medication” form must be filled out and attached to the
doctor’s prescription before medication can be given. The request form is located in the Main Office.
10. First Aid – If a child becomes injured while at school, the child will be taken to the school’s first aid supervisor, who will determine what treatment is
required. If it is determined that medical attention is necessary, Parent will be called. If there is no answer, Parent will be continually called while notifying adults indicated on
Emergency Contact Form. The child will be transported to the nearby Simi Valley Doctors Medial Center, where the child will be treated by an attending physician. Should a
medical emergency arise, paramedics will be summoned. If you do not want this procedure to be followed, Parent must provide Phoenix Ranch with written instructions
indicating what procedures and /or medical treatment the Parent desires. (Children cannot be transported to personal physician)
11. Social Promotion – A Phoenix Ranch School student must have successfully completed the prior grade to advance to the next grade level (see school handbook). The
Phoenix Ranch School does not permit social promotion. Promotion may also be jeopardized by excessive absence (see school handbook).
12. Student Conduct – At the discretion of The Phoenix Ranch School Administrators, students who do not maintain a 2.0 GPA or whose behavior is determined as detrimental
to the program may be suspended or expelled.
13. Parental Behavior and Family Conduct – The Phoenix Ranch School, using administrative discretion, may at any time refuse and/or terminate service due to
parental/guardian behavior, that is deemed detrimental, disruptive, or threatening to staff.
14. Photographs, and Digital and Electronic Media - Pictures of enrolled Phoenix Ranch students may be used in school brochures for promotion. Parents hereby agree
that The Phoenix Ranch School may use photographs of their child for these purposes without requiring prior consent from Parent, and without compensation.
15. Gratuities – No gratuities or expensive gifts may be given to faculty or staff.
16. Bringing “Special Items” to School – Students must leave cellular phones, electronic listening or sending devices, musical instruments (does not refer to musical
instruments required for band), pets and other valuable items at home. If they are brought to campus, they may be confiscated. The parent will be called, and asked to pick up
the item at the end of the school day. When in doubt, check with administration first.

of events.
Payment Plans, Financial Obligations, and Policies
1. Payment Methods: All preschool programs are assessed on month-to-month use:
a. Preschool Monthly Payment Plan – Payments are due on the first of each month and delinquent on the sixth.
b. Preschool Semi-monthly Payment Plan – Payments are due on the first and fifteenth of each month and are delinquent if not received by the fifth and twentieth
respectively. Semi-monthly payments are calculated at a 5% higher rate.
c. Annual Payment Plan—Elect to pay in full by August 1 and receive a 3% discount—full payment is to be made by check or money orders only.
2. Enrollment Withdrawal, Contract Termination, Contract Amendment:
a. Preschool Withdrawal - To withdraw from the preschool program, a one-month written notice in advance must be given. Enrollment for preschool/ prekindergarten
is month to month. If the one-month notice is not given, the parent is responsible for the following month's fee.
b. Contract Amendment - The Phoenix Ranch School contract may not be amended or altered in any way except by a written amendment executed by school
administration. Handwritten or printed alterations written into the Contract, when delivered to The Phoenix Ranch School by parent/guardian, shall be of no legal
force or effect.
1. No Refunds - There is no reduction or refund of fees due to vacations, holidays, absences, illnesses, missed lunches/trips, or other causes.
2. Late Fee - A $15.00 late fee will be assessed for each payment not received by the 5 th of each month.
3. Finance Charge - A finance charge of 1½ % per month is charged on the unpaid balance on payments not received by the 20 th of the month.
4. Returned Checks - Returned checks are charged a $20.00 fee. Checks that do not clear on the second attempt are charged an additional $20.00. NSF checks must be
replaced with cash, money order, or certified check within 5 days.
5. Discount Rates: There are no multiple discounts given and no combining of discounts (the maximum discount given is 10%).
a. Sibling Discount—A 10% discount is given to siblings that reside in the same household. Sibling discounts are applied to the lowest based tuition.
b. Summer Camp Discount for Phoenix Ranch Students—A $2.00 per day discount may be applied to the summer camp day fee for Phoenix Ranch students,
kindergarten through eighth grade. A $2.00 per day camp discount may also be applied to those siblings not attending The Phoenix Ranch School, in the same
household. Camp discounts do not apply to optional programs. Optional camp programs include lunch, extended care, enrichment classes, and trips.
c. Annual School Tuition Discount—A 3% discount is given on tuition only, if payment is paid in full by cash or check by August 1 for the coming school year.
Families paying in full with multiple children receive a 3% discount on the first child and siblings in the same household receive the 10% discount. There are no
multiple discounts given and no combining of discounts (the maximum discount given is 10%).
6. We reserve the right to modify the Credit Card payment option and above policies and procedures.
