This application may be filed with the RTDRS by fax. The RTDRS charges a filing fee <strong>of</strong> $75.00, this may<br />
be paid by VISA or MasterCard only.<br />
Evidence<br />
• The applicant needs to prove the facts <strong>of</strong> their case and may use evidence to support their<br />
claim.<br />
• Any documented evidence you attach to your statement should be marked as “items” (e.g. item<br />
“A”, item “B”, etc).<br />
• Types <strong>of</strong> relevant documented evidence includes: lease or rental agreement, inspection<br />
reports, pictures, receipts, estimates, invoices, utility bills, police reports, health authority<br />
reports, correspondence, etc.<br />

This <strong>Notice</strong> <strong>of</strong> <strong>Hearing</strong> must be served on the other party not less then three (3) business days be<strong>for</strong>e<br />
the date your application is scheduled to be heard. The three (3) days do not include the day the<br />
document is served, the date the application is to be heard, Saturdays, Sundays, or holidays.<br />
When your application is ready <strong>for</strong> filing, YOU must send to the RTDRS (by way <strong>of</strong> fax) the entire<br />
completed application package. The RTDRS will set a date and time <strong>for</strong> your hearing and send the<br />
application package back to you and you will have to make a copy <strong>of</strong> the entire application and<br />
serve it onto your tenant.<br />
