Breaches

These occurred within the High Dependency Unit (9),
Intensive Therapy Unit (3), and Coronary Care Unit (1) at
Queen’s Hospital. All of the breaches occurred due to
delays in the transfer out of patients who had been
‘stepped down’ to general care. This was due to constraints
on the availability of suitable beds for ‘step down’ due to
patient flow delays across the organisation. A root cause
analysis on all patients who breach will commence on the
1st October 2012.The Deputy Director of Nursing during
September randomly spoke to a number of patients who
breached during September and they confirmed that the
reason for the breach was explained to them and that their
privacy and dignity was maintained during this period of
their care.
