% Patients assessed as risk free - This indicator measure the % of patients free from the four harms of 1.Venous Thromboembolism (VTE), 2.category 3 and 4 pressure ulcers, 3.catheter associated urinary tract infections (CAUTI) and 4. falls resulting in harm. The main areas of concern within the trust remain pressure ulcers and falls. These are being addressed by the relevant action plans and significant progress has been made. More detailed analysis is now being undertaken as six months data is now available to identify where the trends for harm are occurring from both a community and hospital perspective. These occurred within the High Dependency Unit (9), Intensive Therapy Unit (3), and Coronary Care Unit (1) at Queen’s Hospital. This is an improvement from August position when 18 single sex breaches were reported. All of the breaches occurred due to delays in the transfer out of patients who had been ‘stepped down’ to general care. This was due to constraints on the availability of suitable beds for ‘step down’ due to patient flow delays across the organisation. A root cause analysis on all patients who breach will commence on the 1 st October 2012. The Deputy Director of Nursing has received feedback from a number of patients who breached in September who were interviewed following the breach and they confirmed that the reason for the breach was explained to them and that their privacy and dignity was maintained during this period of their care.
