3. Quality improvement plan that monitors
compliance with program philosophies,
treatment modalities, and consumer
satisfaction with services.
‣ The facility should have a written QI plan that
describes its purpose and how it involves all
level of staff in identifying problems, creating
proposed solutions, and evaluating the effect
of corrective actions taken.

4. Co-occurring program performance
measures linked to the quality
improvement activities.
‣ The QI efforts need to document
attention to assessing and treating
persons with CODs, outcomes achieved,
problems identified, actions taken to
improve results, evaluation of such
actions, satisfaction or continued actions.
B. COD Screening
The program shall:
1. Develop written procedures for screening for cooccurring
issues. (should be available)
2. Utilize population appropriate screening instruments
that identify both psychiatric and substance use
disorders. (designated instruments with written
instructions for use)
3. Identify staff qualified to provide screening.
(documented qualifications for each staff member)
4. Document staff training on screening procedures.
(evidenced by dates, times, topics, attendance)
All of these must apply to an adolescent population when
that is the population served by the facility.
C. COD Assessment Process
The program shall:
1. Develop written procedures for a strengthbased
assessment process for co-occurring
disorders. (should be available)
2. Utilize assessment instruments that gather
both psychiatric and substance use disorders,
including information re: symptoms of either
set of disorders when the other is at baseline.
(should have rationale for instrument use)

3. Identify how assessment findings are
incorporated into the treatment planning
process. (written description of process)
4. Identify staff qualified to complete the
assessment process for individuals with CODs.
(staff qualifications documented)
5. Document staff training on the co-occurring
disorder assessment process. (dates, times,
outline of training, attendance sheets)
D. COD Program Content
In addition to current licensed activities, the
program shall identify and describe
specific co-occurring disorder services
that, at a minimum, include:
1. Individual and Group Interventions,
2. Skill-Building Interventions,
3. Mental Health and/or Addiction
Education,
4. Medication Education,
