Approval Procedure (continued)
• Within 90 days of the site visit, either:
– A certificate of approval effective for no longer
than one year will be issued that indicates the
facility has met the criteria and is able to
provide co-occurring competent services;
– OR a letter is sent that specifies any areas not
in compliance with the criteria with an
explanation of the corrections required for
compliance.

– Within 3 weeks (15 working days), if in noncompliance,
the facility must submit a plan of
correction, including the action [to be] taken,
timeframe for accomplishment, and person(s)
responsible for ensuring correction, to the appropriate
Department.
– A re-inspection may be conducted for verification.
Following inspection or upon approval of the plan of
correction, the certificate of approval may be issued,
with a letter sent to the facility and to the other
licensing entity for their records.

• The licensing entity will visit the facility at least
annually to conduct a program and clinical
record review for renewal of the certificate of
approval.
• If the facility is licensed by both Departments, a
joint on-site visit may be conducted; if possible,
they will include existing licensing surveys.
• Each approved co-occurring competent facility
listing will be listed in both Department’s Facility
Directory and both Departments’ websites.

A. COD Mission & Philosophy
1. Mission statement & program philosophy
which incorporates an understanding of
the provision of effective COD services
approved by the governing body of the
facility.
‣ The facility should have written
statements that reflect the ideas found in
the “Background” section of the bulletin.

2. Description of intervention strategies that
include consensus & evidence-based
practices for age and culturally
appropriate co-occurring treatment.
‣ The facility should describe the treatment
approaches it is using and provide a
written rationale based in current clinical
research for using these approaches.
