COSIG Co-Occurring Disorders Conference, Hershey PA, May 15-17, 2006 Altoona Regional Health System, Home Nursing Agency, Pyramid HealthCare, White Deer Run/Cove Forge Site Visits, Staff Education, Case Study, Discussion of Data & Prevalence Work Training on the ILSA (Integrated Longitudinal Strength- Based Assessment) Negotiated and endorsed the Consensus Document which included: • Brief background & history of project • 8 Best Practice Principles • CCISC Implementation characteristics • Agreed upon action plans for the County and the Providers Signature on Concensus Document indicated: • Support and participation as a partner in the initiative • Commitment of the organization to adopting the CCISC model • Commitment to participating in a change process consistent with the 8 best practice principles Using the Co-occurring Disorder Consensus Document, developed a work plan which included the following actions: • Design a process & framework to document implementation issues and barriers. • Improve welcoming access and reduce barriers. • Collect & disseminate data & information gathered by project participants. • Evaluate providers co-occurring disorder capability. - the COMPASS tool is used to assess program competencies & develop provider & system level action plans that focus on strategic, incremental, measurable, and sustainable change toward the goal of COD capability. Using the Co-occurring Disorder Consensus Document, developed a work plan which included the following actions (continued): • Develop a short and long term plan to improve the systems ability to provide co-occurring disorder services & supports. - use the CO-FIT tool to monitor and measure system change in implementation of the CCISC. • Assess clinician training needs around the CCISC Best Practice Principles and the core competencies which include Attitudes and Values, as well as Knowledge and Skills. - the CODECAT is used for either supervisory evaluation or clinician self-evaluation of these Competencies. • Accurately identify & report prevalence of co-occurring disorder. In order to implement the work plan, Blair County established the following committee structures: • The Performance Improvement Committee to provide overall management & direction to the project. • The Trainers Group to assure long term viability of the project by offering ongoing provider-level training & support. • The MISA Team to address the clinical & system issues of individuals receiving treatment. 2
COSIG Co-Occurring Disorders Conference, Hershey PA, May 15-17, 2006 To accomplish the work plan activities, the Performance Improvement Committee: • Monitored the completion of the Consensus Document. • Developed Job Scopes for singly licensed agencies. • Designed the communication process between all parties. • Reviews & takes action on issues & barriers identified. • Managed the completion of the COMPASS & reviewed the summary results. • Developed a survey to assess welcoming practices & gather prevalence data. To accomplish the work plan activities, the Trainers Group: – Manages a listing of provider level issues & concerns. – Developed staff & supervisory training opportunities within each provider agency. – Assisted in the completion of the COMPASS. – Plans the process for completion of the CODECAT in each agency. – Uses CDs & VCRs of all trainings offered by ZiaLogic as internal organization training tools. For persons with Co-occurring Disorders, the MISA Team: – Develops service plan recommendations & ongoing consultation – Manages utilization of services for persons with COD – Provide advise & consultation to providers – Identify system barriers to accomplishing the mission & referring those issue to the Performance Improvement Committee. Next Steps • Continue to implement Performance Improvement Committee work plan: – Establish process to assess prevalence – Analyze system’s screening & assessment process • Continue to implement Trainers Group work plan: – Provide ongoing internal training for providers – Establish systemic use of the 8 best practices during regular supervisory situations. Next Steps Update the Consensus Document to include the new partnership with CBHNP (HealthChoices MCO). The revised document will define how the County MH/D&A, Agencies, & CBHNP will work together to implement a quality improvement process that includes the CCISC model, best practice principles, and tools to support on-going incremental system change in the future. 3
