1. This credential is known as the Certified Co-Occurring Disorders Professional (CCDP). The master’s level CCDP is
known as the Certified Co-Occurring Disorders Professional Diplomate (CCDP Diplomate).
2. An official college transcript is required and should be sent directly from the college or university to the PCB Office.
3. If there are problems with your application, you will be notified in writing.
4. This application may be photocopied.
5. When application is approved, applicant will be notified of the written exam dates and locations.
6. Application fee is non-refundable.
7. Use the Checklist in this application to ensure all required materials are included prior to mailing application to the
PCB Office.
