Add Boxes 2 and 4 together and print the<br />

answer in Box 5.<br />

This is the gross payment received for the year. Copy it to Box 12B of your<br />

<strong>return</strong>.<br />

Note<br />

If you’re registered for GST, your gross schedular payment may include<br />

GST. Enter the GST-exclusive amount at Question 12B.<br />

Calculating your <strong>tax</strong> deducted<br />

Add Boxes 1 and 3 together and print the<br />

answer in Box 6.<br />

Subtract Box 6 from Box 5 and print the<br />

answer in Box 7.<br />

This is your total <strong>tax</strong> deducted. Copy it into Box 12A of your <strong>return</strong>.<br />

Note<br />

If you received any other <strong>income</strong> that didn’t have <strong>tax</strong> deducted from<br />

it, print it in Box 23 of your <strong>return</strong>. Write the details of your <strong>income</strong><br />

on a piece of paper and staple it to the top of page 3 of your <strong>return</strong>.<br />

If you’re an ACC client<br />

If you’ve kept all payments and haven’t paid any of the money received<br />

from ACC to your caregiver(s), use the amounts from schedular<br />

payments information on your SOE.<br />

Enter the <strong>tax</strong> on schedular payments in Box 12A and enter the total gross<br />

payments in Box 12B.<br />

If you haven’t received your SOE, call us on 0800 377 774 and we’ll send<br />

you a copy.<br />

If you haven’t provided ACC with your IRD number, please include any<br />

payments that aren’t on your SOE in your IR 3.
