ACC personal service rehabilitation payments<br />

If you are an ACC client or caregiver and received ACC personal service<br />

rehabilitation payments, please read the information on page 61 before<br />

you complete Question 12.<br />

What to show on your <strong>return</strong><br />

Copy the total <strong>tax</strong> deducted (Box 12A) and gross payments (Box 12B)<br />

from your SOE to the same box numbers of your <strong>return</strong>. If you’re an<br />

ACC client or caregiver and received ACC personal service rehabilitation<br />

payments, please read the information on page 61 before you complete<br />

Question 12.<br />

Note<br />

If you’re registered for GST, your gross schedular payment may include<br />

GST. Enter the GST-exclusive amount at Question 12B.<br />

Shareholder-employee salary<br />

If you received a shareholder-employee salary with no PAYE deducted,<br />

show the amount at Question 20.<br />

ACC levies<br />

You’ll have to pay ACC levies on schedular payments. ACC will invoice<br />

you for these.<br />

Question 13 New Zealand interest<br />

Did you receive any New Zealand interest between 1 April 2010 to<br />

31 March <strong>2011</strong> from:<br />

banks<br />

<br />

<strong>Inland</strong> <strong>Revenue</strong><br />

<br />

building and investment societies<br />

<br />

credit unions<br />

<br />

securities<br />

<br />

a partnership, estate or trust<br />

<br />

loans you’ve made?<br />

If so, show all the New Zealand interest you received at Question 13B.<br />

If you were charged commission on any of your interest, claim this at<br />

Question 26. Read the note about expenses on page 36.
