File with county prothonotary office<br />

Report to Center for Local Government Services within 10 days<br />

of effective date of change<br />

Form MS339-submit to county commissioners (contact county<br />

commissioner’s office or PennDOT Municipal Services<br />

Representative for information)<br />

Submit form to State Treasurer Office by March 31 for taxes paid on<br />

fuel <strong>the</strong> previous year.<br />

Certified copy of debt approval proceedings, resolutions and roof of<br />

publication to be filed with Center for Local Government Services<br />

File proposed copy of agreement with Local Government<br />

Commission for review and recommendation, who will comment<br />

within 60 days. Its failure to respond within 60 days will constitute<br />

its approval.<br />

Submit to county board of elections (contact board of elections for<br />

due date and available forms)<br />

Form 990 to PennDOT by September 1 for inclusion in upcoming<br />

year=s Liquid Fuels allocation.<br />

Form 329 to PennDOT prior to spending Liquid Fuels funding on<br />

certain road/street projects.<br />

In boroughs property owners must be given at least 45 days notice<br />

of ordinance requiring sewer connection.<br />

In boroughs and first class townships assessment notice to be sent<br />

to each party assessed within 30 days.<br />

Notify county bureau of elections in writing immediately.<br />

Notify PennDOT of <strong>municipal</strong>ity=s interest in performing winter<br />

maintenance on State-owned roads. The Department will provide<br />

forms.<br />
