The irradiating apparatus is contained in a shielded enclosure which forms an integral
part of the apparatus and under normal operating condition the radiation level outside
the shielded enclosure does not exceed 5 microsievert per hour at a distance of 5 cm
from any readily accessible point on the surface of the apparatus.

照 儀 器 》 牌 照 之 註 冊 醫 生 、 牙 醫 或 放 射韗 技 師韦 親 自 督 導 下 , 方 可 按 法 例 303B 章 第 26 節 規 範 下 操 作 該 輻 照 儀

I understand that the person using the irradiating apparatus as listed in Section E shall be a person who is
holding a valid licence to use the concerned type of irradiating apparatus or is acting under the personal
supervision of a doctor or dentist or radiographer who is holding a valid licence to use the concerned type
of irradiating apparatus in accordance with Regulation 26 of Cap.303B for such purposes. I declare that
to the best of my knowledge the information in this application is true and accurate.

請 注 意 , 每 具 輻 照 儀 器 均 須 個鞄 別 領 有 牌 照 。
Please note that each irradiating apparatus shall be covered by a separate licence.
申 請 人 可 填 寫 表 格頴 DH1271(S) 和 IA-P2 申 請 持 有 輻 照 儀 器 。
You should complete Form DH1271(S) and IA-P2 form for each irradiating apparatus.
2. 輻 照 防 護 物 料頔 Material protection of radiation
根頨 據 輻 射韗 ( 管 制 輻 照 儀 器 ) 規 例 第 17 條 第 (1) 款 規 定 , 在 連 續 使 用 輻 照 儀 器 房 間 外 的 輻 射韗 劑 量 率 , 不 得 超 過 每 小 時頗 3 微 戈 瑞 。
Regulation 17(1) of the Radiation (Control of Irradiating Apparatus) Regulations requires that the radiation dose rate outside the
room where the irradiating apparatus is to be used continuously shall not exceed 3 micrograys per hour.

申 請 費 用 為 $1,150。 當 輻 射韗 管 理 局 收 到 有 關 申 請 後 , 會 向 申 請 人 發 出 一 般 繳 款 單 , 申 請 人 請 依 照 一 般 繳 款
單 內 所 列 的 繳 款 辦 法 繳 付 所 需 費 用 。
假 如 申 請 最 終 被 否 決 , 已 繳 交 的 款 項 將 盡 快 全 數 退 還 。
The ap plication f ee is $1,150. A f t er receivi ng t he application, t he Ra diation Board wi ll issue a
Gene ra l Dem and Note t o t he ap plicant. The a pplicant c ould m ake paym ent accord ing t o t h e pa ym ent
instructions stated in the Gene ral Dem and Note.

(i) 輻 射韗 管 理 局 允 許 申 請 人 持 有 擬 安 裝 及 使 輻 照 儀 器 處 於 可 運 作 狀 態 之 輻 照 儀 器 牌 照 正 本 。
The original copy of the licence permitting you to possess the irradiating apparatus, for installation and testing purposes or
otherwise, while your application for possessing the apparatus in a functional state is being considered by the Radiation
Board.
(ii) A1 段 - 申 請 人 的 身 分 證 / 證 明 文 件 副 本 和 45x60( 毫 米 ) 近 照 兩 張 。
Section A1 - Copy of Identity Card/Document of the applicant and 2 recent photographs of size 45 x 60(mm).
A2 段 - 商 業 登 記 證 和 獲 授 權 人 士 身 分 證 / 證 明 文 件 的 副 本 。
Section A2 - Copy of the Business Registration Certificate and Copy of Identity Card/Document of the authorized
person.
(iii) B 部 - 輻 照 儀 器 的 技 術 規 格頴
Under Section B - Technical specifications of the irradiating apparatus
(iv) C(ii) 部 - 顯 示 防 護 物 料頔 、 輻 照 儀 器 位 置 , 主 輻 射韗 束 方 向 及 毗 鄰 地 方 用 途 的 樓 面 布 置 圖 。
Under Section C (ii) - A floor plan of the premises showing the material protection, location of the irradiating apparatus,
directions of the primary radiation beams and utilization of the adjacent areas.
(v) 輻 照 儀 器 擁 有 權 的 證 明 文 件 ( 例 如 : 發 票 副 本 、 轉 讓 輻 照 儀 器 的 證 明 書頟 )
Documentary evidence to show the ownership of the irradiating apparatus (e.g. invoice copy, proof of transfer of irradiating
apparatus)

注 意 Remarks :
(i) 在 使 用 可 生 產 輻 射韗 影 響 人 體 的 輻 照 儀 器 前 , 申 請 人 須 確 保 使 用 者 必 須 持 有 有 效頒 的 《 使 用 輻 照 儀 器 》 牌 照 , 或 使 用 者 必 須 在 一 位 持 有 有
效頒 《 使 用 輻 照 儀 器 》 牌 照 之 註 冊 醫 生 、 牙 醫 或 放 射韗 技 師韦 在 治 療 / 檢 查 現 場 親 自 督 導 下 , 方 可 進 行 。
Before the use of the irradiating apparatus to produce radiation affecting human body, you should ensure that users of this
irradiating apparatus should hold a valid ‘use’ licence or the users are working under the personal supervision of a registered doctor
or dentist or radiographer holding valid “use” licence who must be present on the premises in which the treatment/examination is
taking place at the time it takes place.
(ii) 表 格頴 中 所 列 的 所 有 有 關 牌 照 必 須 於 呈 交 本 申 請 書頟 時頗 仍 為 有 效頒 。 有 關 牌 照 持 有 人 請 於 牌 照 屆 滿 前 八 個鞄 星 期 把 填 妥 之 申 請 書頟 及 所 需 文 件 交
到 輻 射韗 管 理 局 。 否 則 , 本 局 有 可 能 在 牌 照 屆 滿 前 未 能 完 成 處 理 有 關 牌 照 的 續 牌 申 請 。
Any current licence mentioned in this application must remain valid on the date of submission of this application. The Licensee shall
submit the duly completed application form together with the required documents to the Radiation Board 8 weeks before expiry of
the licence. Otherwise, the Board may not be able to complete processing of the licence renewal application before its expiry.
