申 請 人 可 以 利 用 IA-U2 表 格頴 申 請 牌 照 或 續 牌 , 以 便 獲 准鞔 使 用 輻 照 作 除 照 射韗 人 體 以 外 的 特顠 定 照 射韗 用 途 。 這 些 照 射韗 包 括 非 破 壞 性 測 試 的 工
業 放 射韗 照 相 , 或 對 牲 畜 作 獸 醫 學 放 射韗 照 相 。
You may use form IA-U2 to apply for or renew a licence that permits you to perform radiation exposures using an irradiating
apparatus for specified purposes other than on human bodies. Such purposes may include the performance of industrial
radiography in non-destructive testing, or of veterinary radiography on animals.

申 請 費 用 為 $160。 當 輻 射韗 管 理 局 收 到 有 關 申 請 後 , 會 向 申 請 人 發 出 一 般 繳 款 單 , 申 請 人 請 依 照 一 般 繳 款 單
內 所 列 的 繳 款 辦 法 繳 付 所 需 費 用 。
假 如 申 請 最 終 被 否 決 , 已 繳 交 的 款 項 將 盡 快 全 數 退 還 。

(ii) 申 請 人 的 身 分 證 / 證 明 文 件 副 本
Copy of your Identity Card/Document of the applicant
(iii) 45x60 毫 米 近 照 兩 張 ; 和
2 recent photographs of size 45 x 60(mm); and
(iv) 申 請 人 申 報 的 學 歷 證 書頟 副 本 。
Copy of certificate(s) of your stated qualification(s).
(b) 續 領 牌 照 For renewal of licence (i) 申 請 人 的 45x60( 毫 米 ) 近 照 兩 張 。
2 recent photographs of size 45 x 60(mm).。
4. 表 格頴 中 所 列 的 所 有 有 關 牌 照 必 須 於 呈 交 本 申 請 書頟 時頗 仍 為 有 效頒 。 有 關 牌 照 持 有 人 請 於 牌 照 屆 滿 前 八 個鞄 星 期 把 填 妥 之 申 請 書頟 及 所 需 文 件
交 到 輻 射韗 管 理 局 。 否 則 , 本 局 有 可 能 在 牌 照 屆 滿 前 未 能 完 成 處 理 有 關 牌 照 的 續 牌 申 請 。
Any current licence mentioned in this application must remain valid on the date of submission of this application. The Licensee shall
submit the duly completed application form together with the required documents to the Radiation Board 8 weeks before expiry of the
licence. Otherwise, the Board may not be able to complete processing of the licence renewal application before its expiry.
