<strong>Evaluation</strong> <strong>Form</strong> – <strong>PhD</strong> <strong>Studies</strong><br />

An evaluation made every six months shows if the <strong>PhD</strong> student is progressing satisfactorily in his/her studies<br />

(According to the regulations of the Ministry of Science, Technology and Innovation, and Faculty guidelines).<br />

Supervisors must use this form for the evaluation. Supplementing information concerning the studies can be<br />

submitted on the second page of this form (point 5). When the form has been signed by supervisor, student,<br />

Departmental <strong>PhD</strong> Committee Chairman, and Director of <strong>Studies</strong>, it serves as the Faculty’s official<br />

documentation for the study’s progress. The supervisor ensures that the student signs the completed form and<br />

hands it over to the Departmental <strong>PhD</strong> Committee, who forwards it to the Faculty Secretariat.<br />
