Evaluation Form – PhD Studies
An evaluation made every six months shows if the PhD student is progressing satisfactorily in his/her studies
(According to the regulations of the Ministry of Science, Technology and Innovation, and Faculty guidelines).
Supervisors must use this form for the evaluation. Supplementing information concerning the studies can be
submitted on the second page of this form (point 5). When the form has been signed by supervisor, student,
Departmental PhD Committee Chairman, and Director of Studies, it serves as the Faculty’s official
documentation for the study’s progress. The supervisor ensures that the student signs the completed form and
hands it over to the Departmental PhD Committee, who forwards it to the Faculty Secretariat.
